Objects:By combining modern medicine with traditional medicine,the blood oxygen level-dependent functional magnetic resonance imaging technique was used to observe the distribution of brain functional activation areas during acupuncture in the Shenmai(eight-vessel meeting points pass through the Yang stilts)-Zhaohai(eight-vessel meeting points pass through the Yin stilts)group of acupoints and the Shenmai single acupoint,and to compare the two to study the correlation between the distribution of brain functional activation areas and clinical main treatment;to observe the amplitude of low-frequency fluctuation changes in the resting state after acupuncture in the Shenmai-Zhaohai group of acupoints and the Shenmai single acupoint compared to the resting state before acupuncture,and to study the central mechanism of action of the post-acupuncture effect.Methods:Seventeen people,7 males and 10 females,all right-handed,who met the inclusion criteria,were selected and the subjects were acupunctured at the Shenmai-Zhaohai group of acupoints and the Shenmai acupoint in two trials,with an interval of 7 days between the two trials.A Philips 3.0T dual-source,dual-gradient magnetic resonance scanner was used to collect functional magnetic resonance imaging data before,during and after acupuncture of the right Shenmai-Zhaohai group of acupoints and the right Shenmai acupoint in the subjects,respectively.The image data were processed and statistically analysed based on MATLAB using DPARSFA,SPM12,xj View and Brain Net Viewer software to derive information on the location,voxel values,activation intensity and changes in the amplitude of low-frequency fluctuation before and after acupuncture of the brain activation zone at the Shenmai-Zhaohai group of acupoints and the Shenmai acupoint.Results:1.Positive activation of brain regions induced by acupuncture in Shenmai-Zhaohai group of acupoints included: right hippocampus(BA28),right parahippocampal gyrus(BA36),right insula,right superior temporal gyrus(BA41),right superior marginal gyrus(BA40);Negtive activation of brain regions induced by acupuncture in Shenmai-Zhaohai group of acupoints included: right inferior occipital gyrus(BA19),left middle occipital gyrus(BA18),right superior occipital gyrus(BA19).2.Compared with the resting state before acupuncture,the ALFF values in the Shenmai-Zhaohai group of acupoints after acupuncture were increased in the left orbital inferior frontal gyrus(BA11),left cerebellum-6,left angular gyrus(BA39);Compared with the resting state before acupuncture,the ALFF values in the Shenmai-Zhaohai group of acupoints after acupuncture were decreased in the left medial and paracoxal gyrus(BA24),left supplementary motor area(BA6),right supplementary motor area(BA6),left central anterior gyrus(BA9).3.Positive activation of brain regions induced by acupuncture in Shenmai acupoint included: right cerebellu、 right parahippocampal gyrus(BA36),right precuneus(BA7),left precuneus(BA7),left angular gyrus(BA39)and right angular gyrus(BA39),but no obvious negative activation brain regions.4.Compared with the resting state before acupuncture,the ALFF values in Shenmai acupoint after acupuncture were increased in left precuneus(BA7),left perisylvian cortex;Compared with the resting state before acupuncture,the ALFF values in Shenmai acupoint after acupuncture were decreased in left supplementary motor area(BA6),left precentral gyrus(BA9),left postcentral gyrus(BA3),right inferior temporal gyrus(BA20).5.Differential activation of brain areas induced by acupuncture of the Shenmai-Zhaohai group of acupoints compared to acupuncture of the Shenmai acupoint: positive activation included right fusiform(BA37),right parahippocampal gyrus(BA36),left hippocampus(BA28),right precuneus(BA7),left precuneus(BA7),left orbital middle frontal gyrus(BA11),right dorsolateral superior frontal gyrus,right triangular inferior frontal gyrus(BA45),right superior occipital gyrus(BA19),right paracentral lobule,but no obvious negative activation brain regions.Conclusion:1.Acupuncture of the Shenmai-Zhaohai group of acupoints and acupuncture of the Shenmai acupoint have the same brain areas as well as different brain areas,and the group of acupoints has a wider range of brain activation areas than the single acupoint,presumably related to the synergistic effect of the group of acupoints.2.Acupuncture of the Shenmai-Zhaohai group of acupoints caused activation of several brain regions,including the hippocampus,parahippocampal gyrus,insula,temporal lobe and parietal lobe,which are associated with sleep,cognition,emotion and information processing,and are considered to be the central mechanism for the clinical treatment of insomnia,dementia and anxiety and depression with acupuncture of the Shenmai-Zhaohai group of acupoints.3.When comparing the resting state before and after acupuncture of the Shenmai-Zhaohai group of acupoints and the Shenmai acupoint,the ALFF values in brain areas appear to be reduced and increased,indicating that acupuncture still has an effect on functional brain areas for a period of time after needle extraction,suggesting that acupuncture has an after-effect.
