The Value Of Dual-energy CT And MR Combined With Imageomics In The Diagnosis Of Non-small Cell Lung Cancer Subtypes And The Prediction Of Radiotherapy Effect For Brain Metastasis Of Lung Adenocarcinoma

Objective: To explore the value of dual energy CT-virtual unenhanced CT combined with Radiomics in preoperative diagnosis of squamous cell carcinoma and adenocarcinoma of the lung,and to guide clinical development of individualized treatment plans.Method: A retrospective analysis was conducted on VNCT imaging data of162 patients with pathologically confirmed lung squamous cell carcinoma and lung adenocarcinoma from December 2019 to October 2022,including54 cases of lung squamous cell carcinoma and 108 cases of lung adenocarcinoma.Randomly divided into a training group and a testing group in a ratio of 2:1,using the Darwin Intelligent Research Platform to analyze preoperative DECT(tube voltage 80 and 140 k Vp)VNCT images,delineate regions of interest(ROI),establish a prediction model,and validate the prediction model through receiver operating characteristic curves to evaluate the value of imaging omics features in predicting lung squamous cell carcinoma and lung adenocarcinoma.Result: This study screened 162 patients with lung squamous cell carcinoma and lung adenocarcinoma,including 97 in the training set,32 with lung squamous cell carcinoma,and 65 with lung adenocarcinoma;There were 65 cases in the validation set,including 22 cases of lung squamous cell carcinoma and 43 cases of lung adenocarcinoma.In the training group,the area under the curve value was 0.77,the accuracy was 71.13%,the specificity was 0.75,the sensitivity was 0.69,and the 95% confidence interval was 0.67;In the test group,the AUC value was 0.74,the accuracy was 64.62%,the specificity was 0.59,the sensitivity was 0.67,and the 95%CI was 0.53.A total of 1878 Radiomics features were extracted through the Darwin research platform,and 7 were ultimately retained(3 were related to80 k Vp and 4 were related to 140 k Vp)(P<0.05).Conclusion: Dual energy CT virtual plain scan combined with Radiomics can serve as an auxiliary means to differentiate squamous cell carcinoma and adenocarcinoma of the lung,and has guiding significance for the clinical treatment plan of lung cancer patients.Objective: To explore the value of combining T1 enhanced brain magnetic resonance imaging and T2 WI sequence with radiomics in predicting the effect of stereotactic radiotherapy for brain metastasis of lung adenocarcinoma.Method: A retrospective analysis was conducted on the brain MRI imaging data of 127 patients who underwent stereotactic radiotherapy SRT for lung adenocarcinoma brain metastasis from January 2019 to December 2022.Using the Huiying scientific research platform,manually delineate ROI on T1 weighted enhanced scan and T2 weighted imaging(T2WI)horizontal axis images to extract imaging omics features.Divide the imaging omics features of enhanced scanning combined with T2 WI images into a training set and a validation set in a 4:1 ratio,use the training set to construct the model,and perform internal validation on the corresponding validation set.Analyze the model through ROC curves and calibration curves,and evaluate the predictive performance.Result: This study screened 127 patients with brain metastasis of lung adenocarcinoma who underwent SRT.According to the evaluation criteria for solid tumor efficacy version 1.1,they were divided into an effective group of78 cases and an ineffective group of 49 cases.The training set consists of 77 cases,including 47 cases in the effective group and 30 cases in the ineffective group;There are 50 validation groups,including 29 in the effective group and 19 in the ineffective group.A total of 1407 radiomics features were extracted through the Huiying scientific research platform,and 47 were ultimately retained(P<0.05).The AUC of the training group was0.82,with an accuracy of 0.65,specificity of 0.74,and sensitivity of 0.74;The test AUC was 0.76,accuracy was 0.70,specificity was 0.81,and sensitivity was 0.81.Conclusion: Based on enhanced brain MRI and T2 WI combined with Radiomics,it has good diagnostic value in predicting the effect of SRT in brain metastasis of lung adenocarcinoma.Avoiding excessive treatment and side effects in patients with ineffective radiotherapy,or increasing treatment intensity and changing treatment methods,can provide important information for personalized and accurate diagnosis and treatment decisions.
