| ObjectiveTo investigate the effect of periodontal base treatment on chronic periodontitis(CP)with paroxysmal atrial fibrillation(PAF)evaluation indexes of ventricular remodeling in patients with PAF: P-wave dispersion(Pd)left atrial diameter(PD)LAD),inflammatory indicators: high-sensitivity C-reactive protein(hs-CRP),tumor necrosis factor-α,TNF-α),interleukin-6(IL-6),periodontal parameters: probing depth(PD),bleeding index(BI)and arrhythmia recurrence rate and adverse reaction rate.MethodsA total of 120 patients with chronic periodontitis complicated with paroxysmal atrial fibrillation treated in the Department of Cardiology of the Second People’s Hospital of Lianyungang City from September 2019 to September 2021 were selected as subjects.The subjects were divided into experimental group and control group by random number table method,with 60 cases in each group The experimental group received periodontal basic treatment and atrial fibrillation treatment,while the control group only received atrial fibrillation related treatment.The changes of ventricular remodeling evaluation indexes inflammatory indexes periodontal parameters,arrhythmia recurrence rate and incidence of adverse reactions were compared between the two groups at 6 and 12 months after treatment.ResultsThere was an interaction between intervention and treatment time on the evaluation index,inflammatory index and periodontal parameter changes of ventricular remodeling(P<0.05).6 months after treatment,Pd and LAD decreased significantly from baseline(P<0.05),and 12 months after treatment,there was no significant difference between Pd and LAD in the control group compared with baseline(P>0.05).Compared with the baseline,Pd and LAD in the experimental group decreased significantly,and the differences were statistically significant(P<0.05).During the follow-up period,the inflammatory indexes hs-CRP,TNF-α and IL-6 decreased significantly compared with the baseline,and the differences were statistically significant(P<0.05).At 12 months after treatment,the inflammatory indexes hs-CRP,TNF-α and IL-6 in the experimental group were lower than those in the control group in the same time period,and the difference was statistically significant(P<0.05).6 and 12 months after treatment,the BI and PD parameters of the experimental group were greatly improved,and the difference was statistically significant compared with the baseline and control groups in the same time period(P<0.05).The recurrence rate of arrhythmias in the experimental group 6 and 12 months after treatment was lower than that in the control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups during follow-up(P>0.05).ConclusionPeriodontal primary therapy can not only improve the local periodontal status of patients,but also improve the inflammatory indexes and reduce the recurrence rate of arrhythmia.The experiment has proved that periodontal primary therapy can not only reduce the periodontal parameters,but also reduce the inflammatory indexes It was also found that the recurrence rate of arrhythmia in patients with paroxysmal atrial fibrillation who underwent primary periodontal therapy was much lower than that in patients who did not undergo primary periodontal therapy,suggesting that the recurrence rate of arrhythmia in patients with paroxysmal atrial fibrillation may be related to chronic periodontitis. |
