| Objective: To study the correlation between coagulation function,neutrophil lymphocyte ratio(NLR)and disease severity in patients with acute pancreatitis(AP).Methods: 102 patients with AP treated in our hospital from July 2019 to July 2021 were selected as the observation group.According to the classification standard revised in Atlanta in 2012,all patients can be divided into mild acute pancreatitis(MAP,n=53)and severe acute pancreatitis(SAP,n=49).In addition,51 healthy volunteers who underwent physical examination in our hospital at the same time were selected as the research object and set as the control group(n=51).The age,sex and etiology of the research object were recorded.NLR,coagulation function indexes[prothrombin time(PT),activated partial thromboplastin time(APTT),international normalized ratio(INR),fibrinogen(FIB)] and other related indexes[D-Dimer(D-D),blood platelet count(BPC),serum amylase(AMS)]of MAP and SAP patients in the two groups and the observation group were detected and recorded respectively.The predictive value of NLR,PT,APTT,INR,FIB,D-D,BPC and AMS was analyzed by ROC.Finally,the correlation between NLR,coagulation function and AP severity was analyzed by Person coefficient.Results: NLR,PT,APTT,INR,FIB,D-D and AMS in the observation group were significantly higher than those in the control group,and BPC was significantly lower than those in the control group,the difference was statistically significant(P<0.05).By ROC analysis,the areas under the curve predicted by NLR,PT,APTT,INR,FIB,D-D,BPC and AMS were 0.847,0.756,0.851,0.889,0.798,0.896,0.846 and 0.883 respectively,the difference was statistically significant(P < 0.05).NLR,PT,APTT,INR,FIB,D-D and AMS in SAP patients were significantly higher than those in map patients,and BPC was significantly lower than those in map patients,the difference was statistically significant(P<0.05).Correlation analysis showed that NLR,Pt,APTT,INR,FIB,D-D and AMS were positively correlated with disease severity,the difference was statistically significant(P<0.05).BPC was negatively correlated with disease severity,the difference was statistically significant(P<0.05).Conclusions: Patients with AP will have coagulation dysfunction and systemic inflammatory reaction.The person coefficient analysis shows that NLR,coagulation function indexes and other related indexes are significantly correlated with the severity of AP disease.NLR and coagulation function indexes can be used as predictors of AP developing into SAP,which is of great value in guiding clinical diagnosis and treatment,and is worthy of clinical application. |
