Objective:To investigate the current status and influencing factors of compliance with antihypertensive medication in patients with chronic kidney disease,and to construct a medication reminder application based on the Fogg behavior model.To explore the impact of this application on medication adherence,medication knowledge,rational medication self-efficacy,and medication belief in patients with chronic kidney disease.Methods:1.A survey and analysis of 311 chronic kidney disease patients who were admitted to a tertiary a hospital in Guangzhou from February 2021 to may 2021 were conducted by using convenience sampling,and the general data questionnaire,Morisky Medication Adherence Scale,Medication knowledge scale,Self-efficacy for Appropriate Medication Use Scale,and Beliefs about Medicine Questionnaire were used to conduct the status quo survey.2.Based on the current survey results and Fogg behavior model,the application of medication reminder is designed and developed.3.From June 2022 to October 2022,60 patients with chronic kidney disease admitted to a third-class A hospital in Guangzhou were divided into control group and intervention group with 30 patients in each group.Patients in the control group were treated with routine nursing,and patients in the intervention group were treated with medication reminder application intervention on the basis of routine nursing.The scores of Morisky Medication Adherence scale,Medication knowledge scale,Self-efficacy for Appropriate Medication Use and Beliefs about Medicine Questionnaire were compared between the two groups after 3 months of intervention.Results:1.311 patients with chronic kidney disease scored(5.89 ± 1.58)points in drug compliance,47.6% of them had poor medication adherence,41.2% of them had moderate medication adherence,and 11.2% of them had good medication adherence.2.The medication adherence of patients with chronic kidney disease was positively correlated with medication knowledge,rational medication self-efficacy and medication necessity belief(P<0.05),and negatively correlated with medication anxiety belief(P<0.05).3.The results of ordered logistic regression analysis showed that the consistency of the frequency of taking multiple drugs,blood pressure measurement,health education and medication knowledge were the influencing factors of the compliance of patients with chronic kidney disease(P<0.05).4.The medication reminder application based on Fogg behavior model is composed of three parts: the application management background,the application medical care end and the application patient end.Its main functions include medication management,medication reminder,health education,etc.5.After 3 months of intervention through the medication reminder application,the score of Morisky Medication Adherence scale of patients in the intervention group was higher than that of patients in the control group(P<0.05).There was no difference in Medication knowledge scale,Self-efficacy for Appropriate Medication Use and Beliefs about Medicine Questionnaire(P>0.05).Conclusion: Patients with chronic kidney disease have poor medication adherence,and the consistency of multiple drug use frequency,blood pressure measurement,health education and drug knowledge are the influencing factors of medication adherence.The medication reminder application based on the Fogg behavior model can effectively improve the medication adherence of chronic kidney disease patients with antihypertensive drugs,but it has little impact on patients’ medication knowledge,rational medication self-efficacy,and medication belief. |
