Objective: To study the morphology and characteristics of temporomandibular joint(TMJ)in adolescent patients with skeletal malocclusion by three-dimensional reconstruction. Methods: A total of 148 patients with bony Class I,II,and III malocclusions meeting the inclusion criteria were collected from March 2019 to August 2022 attending the First Affiliated Hospital of Xinjiang Medical Science/Affiliated Dental(University)Hospital,Pediatric Dentistry/Oral Prophylaxis and Orthodontics Department,12-18 years old.The mandibular condyle and articular fossa were reconstructed by three-dimensional reconstruction software, and the volume, surface area, morphological index, articular fossa volume and surface area were obtained the volume, surface area and proportion of condyle in the fossa and the position of condyle in the fossa were analyzed statistically. Results:the volume, surface area and morphological index of bilateral condyles in the patients with skeletal class I malformations were larger than those in the patients with skeletal class II and III malformations(P< 0.05).The size and surface area of condyle were larger in male than in female(P < 0.05).The size and surface area of the articular fossa in the adolescent patients with skeletal class III malocclusion were larger than those in the adolescent patients with skeletal class I malocclusion(P < 0.05).There was a significant difference in the joint space of different skeletal malformations in adolescents. The posterior joint space of skeletal class I malformations was larger than that of skeletal class II malformations(P < 0.05),the supra-articular space and anterior space in patients with skeletal class II malocclusion were larger than those in patients with skeletal class III malocclusion(P< 0.05).The condyle of skeletal type I malocclusion was in the middle position, the condyle of skeletal type II malocclusion was in the posterior position, and the condyle of skeletal type III malocclusion was in the middle position and anterior position. Conclusion: There are significant differences in condylar morphology, position and articular fossa morphology in adolescent patients with bony Class I,II and III malocclusion deformities, and clinical treatment should pay attention to the differences between the different bone facet types to achieve better treatment results.
