Objective:The purpose of this study was to investigate the effect of deep brain stimulation of the subthalamic nucleus(STN-DBS)on freezing of gait in different dopamine-responsive primary Parkinson’s disease(PD),that is,the changes freezing of gait before and after deep brain stimulation.Method:A total of 42 patients diagnosed with primary Parkinson’s disease,accompanied by freezing of gait and receiving bilateral deep brain stimulation of the subthalamic nucleus from the neurosurgery Department of the First Affiliated Hospital of Xinjiang Medical University from October 2020 to August2022 were selected as the research objects.According to their different responses to dopaminergic drugs,the above patients were divided into dopamine-responsive type(24cases),Dopamine-resistant type(17 cases)and dopamine-induced type(1 case).Basic information such as gender,age,duration of disease,type and dosage of oral drugs,Hoehn&Yahr rating(H-Y rating),Montreal Cognitive function score(Mo CA)and so on were recorded.Under the premise that oral dopamine therapy was not changed during the whole process,Respectively in the preoperative and postoperative 3 months by two or more experienced neurologist for patients with Unified Parkinson’s Disease Rating Scale part III(UPDRS-III),freezing of gait questionnaire(FOGQ)score and record.Results:1.Comparison of baseline data: There was no statistically significant difference in gender,age,duration of disease,preoperative H-Y grading,Mo CA,FOGQ score and other baseline data between the dopamine response group,the dopamine resistance group and the dopamine-induced group(P > 0.05),indicating comparability.2.There were statistically significant differences in UPSRS-III and FOGQ in the dopamine response group and the dopamine resistance group 3 months after surgery compared with those before surgery(P< 0.05).Conclusion:The subthalamic deep brain stimulation has a certain effect on the freezing of gait of dopamine-responsive,dopamine-resistant and dopamine-induced PD,and the effect on dopamine-responsive PD is better.
