Objective:To investigate the relationship between TSH and different pregnancy complications in twin pregnancy.Methods:From January 1,2019 to November 23,2022,199 pregnant women with twin pregnancy who had regular prenatal examination and delivery in the First Affiliated Hospital of Xinjiang Medical University were selected.Exclude 11 cases that did not meet the inclusion criteria.The pregnancy complications related to TSH level in early pregnancy were analyzed,and then divided into two groups according to whether the pregnancy complications occurred or not.The general data were analyzed to find the factors with the highest correlation,and the independent influencing factors were analyzed by binary logistic regression.Independent risk factors were selected and ROC curve analysis was performed to compare the area under the curve and the cut-off value.Result:(1)There were statistically significant differences in TSH values in the first trimester between the two groups with or without GDM and between the two groups with or without hypertensive disorder complicating pregnancy.(2)There were significant differences in pre-pregnancy body weight,pre-pregnancy(BMI),early pregnancy TSH and early pregnancy TPOAb between the two groups with or without GDM(P<0.05).There was a significant positive correlation between pre-pregnancy BMI,early pregnancy TSH and GDM(P<0.05)and they were independent risk factors for GDM(OR>1).ROC curve analysis showed that TSH in early pregnancy had a good effect on predicting GDM,and the corresponding cutoff value was 3.600 m IU/L.(3)There was a significant difference in the history of adverse pregnancy and early pregnancy TSH between the two groups(P<0.05),and there was a significant positive correlation(P<0.05),and it was an independent risk factor for hypertensive disorder complicating pregnancy(OR>1).ROC curve analysis showed that TSH in early pregnancy had a good effect on predicting hypertensive disorder complicating pregnancy,and the corresponding cutoff value was 3.045 m IU/L.Conclusion:TSH in the first trimester of twin pregnancy and pre-pregnancy BMI are independent risk factors for GDM.When TSH≥3.6m IU/L in the first trimester,GDM is more likely to occur.Adverse pregnancy history of twin pregnancy and TSH in early pregnancy are independent risk factors for hypertensive disorder complicating pregnancy.When TSH≥3.045 m IU/L in early pregnancy,hypertensive disorder complicating pregnancy is more likely to occur. |
