| Objective: To analyze the correlation between portal vein pressure and noninvasive examination indexes and HPVG in diagnosis of PH.Methods: From January 2022 to December 2022,45 patients with portal hypertension of cirrhosis complicated with digestive tract hemorrhage were selected from Xinjiang Uygur Autonomous Region People’s Hospital and divided into bleeding group and no bleeding group according to no gastrointestinal tract hemorrhage before operation.2D-SWE,imaging and serological examinations were performed in both groups before surgery,and HVPG and PVP were determined intraoperatively.Results:(1)The correlation between HVPG and PVP was statistically significant(P<0.05),and was positively correlated,with the correlation coefficients of 0.9and 0.56 in bleeding group and non-bleeding group,respectively.(2)The correlation analysis between LS and SS and HVPG in bleeding group was statistically significant(P<0.05),and was positively correlated,while the correlation analysis between LS and SS and HVPG in non-bleeding group was not statistically significant(P>0.05).(3)The correlation analysis between TB and HVPG in both groups was statistically significant(P<0.05),and was positively correlated;the correlation coefficients of the bleeding group and the non-bleeding group were 0.5 and 0.64,respectively;the correlation analysis of ALB,PT and PLT count and HVPG in the bleeding group was statistically significant(P<0.05),and was negatively correlated.The correlation coefficients were-0.55,-0.51 and-0.50,and there was no statistical significance between the ALB,PT and PLT count and HVPG in the non-bleeding group(P>0.05).(4)There was a positive correlation between LS and SS and PVP.The correlation coefficients of LS and SS in bleeding group were 0.62 and 0.58,while those in non-bleeding group were both 0.46,with statistical significance(P<0.05).(5)PBFV,TOS and SL were weakly correlated with PVP,and PBFV was negatively correlated with PVP,with the correlation coefficients between bleeding group and non-bleeding group being-0.48 and-0.46.TOS and SL were positively correlated with PVP,and the correlation coefficients were 0.48 and 0.43 in the bleeding group and 0.47 and 0.41 in the non-bleeding group,and the correlation coefficients in the bleeding group were stronger than those in the non-bleeding group.(6)HVPG,LS,SS,TB,ALB,PT,PLT count and PBFV’s ROC area below the line were 0.525,0.248,0.585,0.575,0.483,0.692,0.553 and 0.440,in which the sensitivity of HVPG and PLT count was 96% and 92%,respectively.The specificity of LS was higher(89%).Conclusion: The non-invasive test has obvious correlation with HVPG and PVP,which can be used as an important auxiliary method to diagnose PH.Among them,HVPG,PT,TB,SS and PLT technologies have a certain value in predicting EVB. |
