Objective: To investigate the effect of songchi ointment combined with running platform exercise on joint synovial tissue morphology,joint range of motion,interleukin 17,inducible nitric oxide synthase,transforming growth factor-beta1,von willebrand factor,vascular endothelial growth factor and their mechanism in joint fibrosis of rats with joint contracture.Methods: Among 44 Wistar rats,8 rats were randomly selected as the normal control group(NC group),and the other 36 rats were replicated according to Nagai method.Thirty-two rats were divided into model control group(MC group),songchi ointment intervention group(SC group),running platform exercise rehabilitation intervention group(RE group),and songchi ointment + running platform exercise intervention group(SR group)according to random number table method,8 rats in each group.NC group was not given hind limb fixation,no intervention;In the MC group,the rats moved freely in the cage without any intervention.SC group was given medicated massage once a day,6 days a week;RE group was given treadmill training,once a day,6 days a week;SR group was given treadmill training and then massage with medicine,once a day,6 days a week.After 3 weeks and 42 days of intervention,HE staining was performed on the synovium of each group and observed under the optical microscope.X-ray images were taken before intervention and 42 days after intervention,and joint motion(ROM)was measured by Image J software.The contents of IL-17,i NOS and v WF in serum samples and VEGF and TGF-β1 in synovial fluid were detected by enzymolinked immunoassay(ELISA).Results: 3 weeks after the model of joint contracture was reproduced,the inner wall of synovial membrane was atrophied and thickened,and a large number of inflammatory cells were infiltrated.After42 days of intervention,the synovial tissue of SR group was arranged neatly without thickening,and a few inflammatory cells infiltrated.Before intervention,ROM in model group was significantly decreased compared with NC group(P < 0.05).After 28 days and42 days of intervention,ROM in SR group and SC group was significantly increased compared with MC group,the differences were statistically significant(P < 0.05).Compared with 28 days before intervention,28 days after intervention,42 days before intervention,and 42 days before intervention,the mean ROM of the five groups at 28 days after intervention increased compared with that before intervention,and the ROM of the five groups showed a trend of steady improvement.After intervention,compared with MC group,the contents of IL-17,i NOS,v WF,VEGF and TGF-β1 in SC,RE and SR groups were significantly decreased,and the contents of IL-17,i NOS,VEGF and TGF-β1 in SR group were significantly lower than those in SC and RE groups(P < 0.05).Conclusion:Songchi ointment combined with running platform exercise rehabilitation intervention can cooperatively improve the range of motion,reduce the release of inflammatory factors,and reduce the secretion of vascular endothelial factors related to fibrosis in rats with joint contracture,thus improving the degree of joint fibrosis in rats with joint contracture. |
