Objective: By analyzing the curative care expenditure of diabetes diseases in certain city of northern China in 2019,this study evaluates the distribution of the expenditure of diabetes from the dimensions of institutional flow,service function and population flow of the expenses.Which can not only understand the overall scale of the curative care expenditure from a macro perspective,but also analyze and compare the components of the curative care expenditure from a micro perspective,so as to evaluate the economic burden of different patients,and provide policy advice for the prevention and expenditure of diabetes and accurate expenditure control.Methods: This study adopts a multi-stage stratified random sampling method to extract outpatient and inpatient expense data of various health institutions at all levels in certain city of northern China in 2019.Based on the "SHA 2011" calculation principle,the apportionment method from the total to individual cases is adopted.Use Stata 15.0 to clean and establish the database and calculate the cost of diabetes treatment.From the aspects of service function,institutional distribution,beneficiary population,etc.,we can fully understand the occurrence of diabetes treatment cost;at the same time,we will conduct single-factor,multi-factor and gray correlation analysis on the factors that affect hospitalization costs.Results:1.Overall,the cost of diabetes treatment in certain city of northern China in 2019 was 829 million yuan,of which 391 million yuan(47.23%)was spent on outpatient treatment and 437 million yuan(52.77%)was spent on inpatient treatment.The average cost of treatment is 2327.27 yuan,the average cost of outpatient service is228.2 yuan,and the average cost of hospitalization is 9710.13 yuan.2.From the perspective of gender and age,the treatment cost of female patients is 392.489 million yuan,and that of male patients is 436.562 million yuan.The cost of treatment is mainly concentrated in the elderly population,and the highest proportion of expenditure is in the 60-64 age group(136.4614 million yuan).The cost of female patients has been increasing from the age of 15 to 69;the cost of male patients has been increasing rapidly from the age of 0 to 64.3.The average outpatient fee for male patients is 236.66 yuan,and the average hospitalization fee is 9410.01 yuan;the average outpatient fee for female patients is219.22 yuan,and the average hospitalization fee is 10748.46 yuan.From the perspective of average outpatient expenses per visit,it can be seen that the average outpatient expenses per visit for patients aged 0-19 and over 85-95 is much higher than that of other age groups,which can reach 2-4 times that of other age groups.The average outpatient expenses for each age group fluctuated less,mainly around200-220 yuan.Among them,the average outpatient fee for children aged 5-9 was the highest(725.43 yuan).4.In terms of institutional distribution,the cost of diabetes treatment mainly occurs in hospitals,and the total cost is 648.6559 million yuan,accounting for78.24% of the total treatment cost.Among the hospital expenses,general hospitals accounted for the highest proportion,accounting for 91.68% of the total hospital treatment expenses,accounting for 71.73% of the total treatment expenses,followed by comprehensive medical and health institutions,and public health institutions accounted for the lowest proportion.5.The highest average outpatient expenses and hospitalization expenses are specialized hospitals,which are 757.56 yuan per outpatient visit and 11148.43 yuan per hospitalization expense.6.Through the single factor analysis of non-parametric test,it was found that age,length of hospitalization,surgery,insurance type institution level and the presence or absence of complications all have an impact on hospitalization expenses;through the analysis of gray correlation model,it was found that it affects the cardiovascular and cerebrovascular diseases in Dalian in 2019 The factors of the average hospitalization expenses per disease are drug expenses and examination and laboratory expenses.Conclusion: The burden of diabetes in Dalian is heavy and consumes a lot of health resources.The overall scale of treatment costs is large,and the costs are mainly concentrated in hospitals,and the proportion of public health institutions is relatively low.The treatment cost of male patients is higher than that of women;the treatment cost is mainly concentrated in the elderly group,and the high outpatient and hospital costs are concentrated in the high age group(≥60 years old).The main factors affecting the hospitalization expenses of patients are age(negative correlation),length of stay(positive correlation),and institution level(positive correlation).
