Background:The prevalence of postpartum depression in perinatal women is relatively high,ranging from 0.5% to 60.8% globally and 6.5% to 43.1% domestically.The actual prevalence may be higher than the available data for a variety of reasons.Postpartum depression can lead to the serious consequences of self-injury or even suicide.The occurrence of postpartum depression will have a negative impact on the healthy growth of newborn and there is a positive correlation between the occurrence of postpartum depression in women and their husbands.Therefore,it is very important to pay attention to the mental health status of perinatal women and carry out related research.In this study,prenatal mindfulness interventions developed and implemented for this special group was designed to provide a scientific basis for the prevention of postpartum depression in perinatal women.Objective:This study preliminarily established a prenatal mindfulness intervention program based on the Internet,and discussed the preventive effect of this intervention on postpartum depression,with a view to reducing the risk of postpartum depression in perinatal women,thereby contributing to the promotion of maternal and infant physical and mental health,and laying a theoretical foundation for the prevention and treatment of postpartum depression.Method:Women in the third trimester of pregnancy who underwent routine birth examination and voluntarily accepted intervention in the obstetrical department of the First Affiliated Hospital of China Medical University from December 2021 to October 2022 were selected as research objects,and randomly divided into the intervention group and the control group by random number table method,with 50 cases in each group.The control group received routine birth examination and health education,and the intervention group received web-based prenatal mindfulness intervention measures on the basis of the control group.Edinburgh Postpartum Depression Scale,Generalized Anxiety Scale,Perceived Stress Scale,Pittsburgh Sleep Quality Index Scale and Five Facet Mindfulness Questionnaire were used to evaluate the changes in postpartum depression,anxiety,perceived stress,sleep quality and mindfulness level of the subjects in the two groups before the intervention and at 4 weeks postpartum.Both groups were assessed for postpartum depression,anxiety,perceived stress,sleep quality and levels of mindfulness.SPSS25.0 software was used for data analysis.T-test and Chi-square test were used to compare the differences in baseline characteristics between the two groups,independent sample T-test was used to compare the differences in scores between the two groups after intervention,and paired sample T-test was used to analyze the changes in scores in each group before and after intervention.P < 0.05 was considered as statistically significant.Results:1.Before intervention,there was no statistical significance between the intervention group and the control group in the health questionnaire,Edinburgh Postpartum Depression Scale(EPDS),Generalized Anxiety Scale(GAD-7),Perceived Stress Scale(PSS),Pittsburgh Sleep Quality Index Scale(PSQI)and Five Facet Mindfulness Questionnaire(FFMQ)(P > 0.05).It is comparable.2.Before intervention,the total scores of EPDS,GAD-7,PSS,PSQI,FFMQ and scores of each dimension were not statistically significant between the intervention group and the control group(P > 0.05),indicating that the scores of the two groups in depression,anxiety,perceived stress,sleep quality and mindfulness level were comparable before intervention;after the implementation of mindfulness intervention,the total score of FFMQ of the intervention group was significantly higher than that in the control group(P< 0.05),indicating that the level of mindfulness in the intervention group was significantly improved compared with the control group,while there was no statistical significance in EPDS,GAD-7,PSS,PSQI and FFMQ between the two groups(P > 0.05).3.The incidence of postpartum depression and EPDS scores before and after intervention were compared between the two groups.There were 8 new cases(19.0%)in the intervention group and 20 new cases(44.4%)in the control group.The incidence of postpartum depression in the intervention group was statistically significant compared with that in the control group(P < 0.05).The results showed that the incidence of postpartum depression was improved in the intervention group compared with the control group.The EPDS scores of the two groups before and after intervention were compared within the group,and the intervention group had statistical significance(P < 0.05),while the control group had no significant statistical difference(P > 0.05).After the mindful emotion management intervention,the EPDS score of the intervention group decreased significantly,while the EPDS score of the control group increased slightly,indicating a significant improvement in the level of depression in the intervention group.4.The GAD-7 scores of the two groups before and after intervention were compared within the group,the intervention group had a statistically significant difference(P <0.05),while the control group had no significant statistical difference(P > 0.05).The GAD-7 score of the intervention group decreased after the intervention,while the GAD-7score of the control group increased,indicating that the anxiety level of the intervention group was significantly improved through the implementation of mindfulness intervention.5.The PSS scores of the two groups before and after intervention were compared within the group,and the difference between the two groups before and after intervention was statistically significant(P < 0.05).The PSS score of the intervention group decreased after intervention,while the PSS score of the control group increased after intervention,indicating that the perceived stress level of the intervention group was significantly changed compared with that of the control group.6.The PSQI scores of the two groups before and after intervention were compared within the group,and there was no statistical significance between the two groups(P > 0.05),suggesting that mindfulness intervention did not significantly improve the postpartum sleep quality of pregnant women.7.The total FFMQ scores and the scores of each dimension of the two groups before and after intervention were compared within the group.After 4 weeks of intervention,the total score of FFMQ and the scores of "description" and "conscious action" in the intervention group had statistical significance(P < 0.05),but there was no significant change in the scores of "observation","nonjudgment" and "nonreactivity" in the intervention group,while the total score of FFMQ and the scores of each dimension in the control group had no significant change in the control group(P >0.05),indicating that the level of mindfulness of pregnant women in the intervention group was significantly improved after mindful emotion management intervention,mainly in the dimensions of "description" and "conscious action".Conclusion:This study found that prenatal mindfulness intervention based on the network can effectively prevent the occurrence of postpartum depression in perinatal women,and has a positive effect on improving postpartum anxiety,perceived stress and improving the level of mindfulness,suggesting that clinical mental health education can effectively prevent the occurrence of postpartum depression and postpartum negative emotions in perinatal women through the implementation of prenatal mindfulness intervention measures.
