| Objective: To investigate the effects of anterior-posterior approach and simple posterior approach on the clinical curative effect,quality of life and postoperative complications of severe multi-level cervical spondylotic myelopathy.To optimize the surgical treatment of patients with severe multilevel cervical spondylotic myelo pathy.Methods:In this paper,120 cases of multi-segment cervical spondylotic myelopathy were treated with simple posterior approach and combined posterior approach in our hospital from January 2016 to December 2021.Postoperative neurological recovery,quality of life,postoperative complications and satisfaction were assessed.Results: JOA,JOA recovery rate,PF,RP,GH and satisfaction have no significant difference between the two groups at each follow-up time point.Patients who unde rwent combined anterior-posterior approaches had higher NDI at 3 and 12 months postoperatively(P< 0.05),and higher VRS NRS at 12 months postoperatively(P<0.05).Compared with the simple posterior approach,the patients in the combined anterior-posterior approach had higher incidence of postoperative axial symptoms(P< 0.05),as well as operation time,intraoperative blood loss,postoperative hospital stay and hospitalization cost(P < 0.001).Conclusions:There was no significant advantages in neurological outcomes,quality of life,and satisfaction with combined anterior and posterior surgery,but bringsgr eater surgical trauma,hospitalization cost and complications. |
