Objective:Micro RNA is associated with the pathogenesis and prognosis of hepatocellular carcinoma(HCC),and this study aims to explore the clinical value of the combined detection of seven miRNAs(miR-122,miR-192,miR-21,miR-223,miR-26a,miR-27a and miR-801)in plasma for primary hepatocellular carcinoma.Methods:Real-time polymerase chain reaction(q RT-PCR)was used to detect the expression levels of 7 miRNAs in plasma of 36 HCC patients and 41 patients with chronic hepatitis B(CHB)and liver cirrhosis(LC).X~2 test was used to compare the expression differences of seven miRNAs in plasma of HCC patients,hepatitis and liver cirrhosis patients and their clinicopathological relationship with HCC patients.The receiver operating characteristic curve(ROC)was drawn to evaluate the practical application value of seven kinds of miRNA and AFP in plasma for the diagnosis of HCC.The survival curve was plotted to analyze the relationship between the expression levels of seven miRNAs in plasma and the survival of HCC patients.P<0.05 was statistically significant.Results:1.The positive rate of 7 miRNAs in plasma of HCC patients was 69.4%(25/36),and the positive rate of 7 miRNAs in plasma of patients with hepatitis cirrhosis was 24.4%(10/41),with a significant difference(P<0.001).2.The sensitivity of 7 miRNAs in plasma was 69.4%,the specificity was 75.6%,and the area under curve(AUC)was 0.814,while the sensitivity of AFP was 55.6%,the specificity was 80.5%,and the AUC was 0.781,the difference was statistically significant(P<0.001).3.There were no significant differences between the expression levels of the seven miRNAs in plasma and the clinicopathological features such as age,sex,smoking history,drinking history,and alpha-fetoprotein expression levels.4.There was no significant difference in overall survival(OS)time among HCC patients(P=0.7901).The disease-free survival(DFS)time of patients with positive expression of seven miRNAs in plasma was significantly shorter than that of negative patients,and the difference was statistically significant(P=0.0152).Conclusion:1.The expression level of 7 miRNAs in plasma is related to hepatocellular carcinoma,and is higher than that of hepatitis and cirrhosis.2.Compared with AFP,7kinds of miRNAs in plasma have higher sensitivity to HCC diagnosis,which can complement AFP and improve diagnostic efficiency.3.The expression level of 7 miRNAs in plasma was not related to the clinicopathological characteristics such as age,gender,smoking history,drinking history,and AFP expression level of the patient.4.The expression of 7 miRNAs in plasma can predict the risk of HCC recurrence,which may be a potential biological indicator of poor HCC prognosis.
