Objective: The purpose of this study was to compile a questionnaire to assess the level of proactive behavior in lung cancer prevention among high risk groups,and to test the reliability and validity of the questionnaire.Methods: Firstly,the information-motivation-behavioral skills model was used to determine the dimensions of proactive behaviors for lung cancer prevention in highrisk groups of lung cancer.Secondly,the item pool of the questionnaire was constructed through literature review,and the semi-structured interview method was used to summarize and supplement the items under the framework of the theoretical model according to the interview results.The first draft of the proactive behavior assessment questionnaire for lung cancer prevention in high-risk groups of lung cancer was formed.A questionnaire survey was conducted among 267 people at high risk of lung cancer who met the inclusion and exclusion criteria by convenience sampling method.The questionnaire items were screened by correlation coefficient method,critical ratio method and internal consistency coefficient method.The final version of the questionnaire was formed through reliability and validity test.Results: 1.Formation of the initial questionnaire: a semi-structured interview was conducted with 11 high-risk groups of lung cancer.Combined with literature review and qualitative interview results,a lung cancer prevention proactive behavior questionnaire for high-risk groups of lung cancer was initially formed with 4dimensions and 46 items.2.Delphi expert consultation: a total of 18 experts were invited to participate in the consultation.The expert authority coefficients of the two rounds of consultation were0.92 and 0.90,and Kendall’s W values were 0.167 and 0.246,respectively.The pre-test version of the questionnaire formed after expert consultation included 4 dimensions and38 items.3.Validity of the questionnaire: I-CVI ranged from 0.800 to 1.000,S-CVI/Ave was0.970.After project analysis and exploratory factor analysis,a total of 4 common factors were extracted,which could explain 56.571% of the variation.4.Reliability of the questionnaire: the Cronbach’s α coefficient of the total questionnaire was 0.897,and the Cronbach’s α coefficient of each dimension was 0.743 to 0.875.The test-retest reliability was 0.832,and the test-retest reliability of each dimension ranged from 0.758 to 0.882.Conclusion: In this study,the lung cancer prevention initiative behavior questionnaire for high-risk groups of lung cancer was developed,with 23 items in 4 dimensions,including lung cancer risk cognition(7 items),preventive behavior motivation(4 items),preventive behavior skills(7 items),and active prevention behavior(5 items).The questionnaire has good reliability and validity.
