| Objective: To explore the correlation between different degrees of pelvic organ prolapse(POP)and subjective symptoms and objective indicators of bladder storage and excretion function.Further to explore the diagnostic value of POP-Q parameters and(Ba-Aa)values for POP combined with stress urinary incontinence(SUI)and POP combined with bladder outlet obstruction(BOO).Methods: A total of 377 patients diagnosed with pelvic organ prolapse admitted to our hospital from July 2021 to July 2022 were selected.Among them,74 cases were POP combined with SUI,and 71 cases were POP combined with BOO.In addition,77 cases were simple POP without subjective symptoms and abnormal objective indicators.All cases collected information including: POP-Q stage,POP-Q parameters,subjective lower urinary tract symptoms,UDI-6 questionnaire,and urodynamic parameters.Analyzed the difference and correlation of the collected data.ROC curve was used to explore the predictive value of relevant parameters for diseases.Results: With the severity of POP,the proportion of patients with subjective lower urinary tract symptoms increased.POP-Q stages were positively correlated with the severity of subjective lower urinary tract symptoms(r=0.671,P<0.01).There were statistical differences between different POP-Q stages and urodynamic parameters(P<0.05).Different POP-Q scores were negatively correlated with maximum cystometric capacity,volume at first desire,volume at strong void,and maximum flow rate(r=-0.746,r=-0.779,r=-0.698,r=-0.763,P<0.01),and positively correlated with residual urine volume and detrusor pressure at maximum urine flow rate(r=0.839,r=0.564,P<0.01).There were statistical differences in the Aa and Ba and the value of(Ba-Aa)between POP with SUI and simple POP(P<0.05),Aa and Ba and the value of(Ba-Aa)had diagnostic value for POP with SUI.Aa,Ba,C,Ap,Bp,D and(Ba-Aa)values were statistically different between POP with BOO and simple POP(P<0.05).Aa,Ba,C,Ap,Bp,D and(Ba-Aa)values had diagnostic value for POP with BOO.Conclusion: The degree of pelvic organ prolapse is correlated with subjective symptoms,objective urodynamic parameters of bladder storage and excretion functions.POP-Q stages can provide clinical basis for predicting bladder storage and excretion functions before and after surgery.POP-Q indicates point parameters can provide an important reference for the clinical diagnosis of POP with SUI and POP with BOO.Meanwhile,it can provide clinical guidance for the selection of surgical methods and individualized diagnosis and treatment. |
