| Objective: Ulcerative Colitis(UC)is one of Inflammatory Bowel Disease(IBD).Clinically,it was often manifested as chronic recurrent abdominal pain,diarrhea,mucolytic pus and blood stool.The incidence and prevalence rate increased year by year.The risk of colon cancer is also significantly increased.Pyroptosis is closely related to the incidence of UC,and clinical studies on the expression of pyroptosis related proteins in the tissues of UC patients are relatively rare.In order to further study the expression of pyroptosis related proteins in the colon mucosa of UC patients and its relationship with inflammation,We measured the NOD-like receptor thermoprotein domain 3(NLRP3),interleukin-1β(Interleukin),which is associated with pyrokinosis The expression of-1β,IL-1β)and cysteine aspartic enzyme 1(Caspase-1)in the colonic mucosa of UC patients with ulcerative colitis was compared,and the role and correlation of the above three pyroptosis related proteins in the pathogenesis of UC were discussed.Methods: Colonic mucosa specimens of 58 UC patients were collected from December 2018 to July 2021 at Hainan Provincial People’s Hospital,including 30 males and 28 females.According to the clinical data,laboratory examination and endoscopic findings,27 cases were mild(15males and 12 females)and 31 cases were moderate to severe(15 males and 16 females).At the same time,55 patients with colorectal cancer who underwent surgical resection and pathological tissue biopsies of normal intestinal wall were collected as the control group,including 28 males and27 females.There was no significant difference in gender/age between the two groups(P>0.05).All patients’colonic tissue specimens were obtained from the paraffin-embedded tissue specimens of colonoscopic biopsy maintained by the Department of Pathology,Hainan Provincial People’s Hospital.The selected control specimens were confirmed as normal tissues adjacent to cancer by pathological examination.The expressions of pyroptosis-related proteins NLRP3,IL-1βand Caspase-1 in intestinal mucosa of all patients were detected by immunohistochemical SP method,and then the differences between the two groups and the correlation between pyroptosis-related proteins were compared by statistical methods.Results: 1.There were 17 cases of grade I,31 cases of grade II,and10 cases of grade III,as determined by Riley-Mani-methodology.The mean and standard deviation of the average optical densities of the NLRP3,IL-1β,and Caspase-1 proteins in the colonic mucosa of UC were 0.15±0.04,0.25±0.86,The samples of 55 control patients were confirmed as normal paracancular tissues by pathological examination.The average optical density values of NLRP3,IL-1β and Caspase-1 protein in colonic mucosa were 0.03±0.02,0.04±0.30 and 0.04±0.03,respectively.The expression of NLRP3,IL-1β and Caspase-1 in colonic mucosa inflammation tissues were positive,while those in the control group were negative or weakly positive.The expression of NLRP3,IL-1β and Caspase-1 in colon mucosa of UC patients were significantly higher than those of the control group(P<0.001).Immunohistochemical detection of the expression of NLRP3,IL-1β,Caspase-1 in intestinal mucosa of each group: Compared with the control group,the expression of NLRP3,IL-1β,Caspase-1 in intestinal epithelial tissue of ulcerative colitis patients had higher average integral optical density of brownish yellow granules in IHC section of colon tissues,and the staining intensity was enhanced,and more brown granules were visible.There was statistical significance in expression(P<0.001).There was a positive correlation between UC pathological grades and IL-1βprotein expression(r=0.402,P<0.01).The expression of IL-1βprotein was significantly different between pathological grade I and grade II(P<0.05),between pathological grade I and grade III(P<0.05),but not between pathological grade II and grade III(P>0.05).There was no correlation between the expression of NLRP3 protein and pathological grades,but there was no significant difference between pathological grades I and II(P>0.05),between pathological grades I and III(P>0.05),and between pathological grades II and III(P>0.05).There was no significant difference in Caspase-1 expression between pathological grade I and II(P>0.05),between pathological grade I and III(P>0.05),and between pathological grade II and III(P>0.05).2.According to the results of the correlation analysis,there was a positive correlation between the expression of the proteins IL-1,NLRP3 and Caspase-1 in the UC colonic mucosa,NLRP3 protein and Caspase-1 in ulcerative colitis were positively correlated(r = 0.882,P <0.001),The expression of Caspase-1 protein and IL-1 protein were positively correlated(r= 0.825,P<0.001).The expression of the NLRP3 protein and the IL-1 protein were positively correlated(r = 0.761,P <0.001).Conclusion: 1.IL-1β,NLRP3 and Caspase-1 were mainly expressed in epithelial cells and inflammatory cells of intestinal mucosa.2.Compared with the control group,the expression levels of IL-1β,NLRP3 and Caspase-1 in the intestinal mucosa of UC patients were significantly increased.3.IL-1β is closely related to the occurrence and development of UC inflammation.NLRP3 and Caspase-1 may participate in the inflammatory response and immune regulation of UC,mediate pyroptosis,thereby affecting the occurrence and development of UC inflammation,suggesting that pyroptosis may be involved in the pathogenesis of UC. |
