Objective:Based on the social learning theory,the early activity program of total hip arthroplasty(THA)patients was developed,and the effect of intervention on the early rehabilitation of hip function in THA patients was evaluated,providing reference for the joint function rehabilitation of clinical patients with THA.Method:This study is divided into four parts.The first part is to discuss the research background and significance of THA early activity program through literature review.Part II: Through literature review and group discussion,this paper discusses the interaction mode of early activities from the perspective of three-way interaction theory,observational learning theory,self-regulation theory and self-efficacy theory of social learning theory.Based on this,it develops specific contents combining literature review and group discussion,and finally determines the intervention plan based on social learning theory through expert correspondence consultation.Part III:According to the inclusion and exclusion criteria,62 patients from a third-grade Grade-A hospital from January 2022 to October 2022 were selected as research objects(patients undergoing unilateral THA).Convenience sampling method was adopted,non-synchronous control was adopted,and the first 31 patients were included in the control group and the last 31 patients in the experimental group according to the time sequence of patient admission.The control group was given routine nursing program after orthopedic surgery,and the experimental group was given early activity program based on social learning theory after surgery,a total of14 days of intervention.Harris hip function rating scale,rehabilitation exercise selfefficacy rating scale and Kinetophobia Tampa scale were collected within 24 hours after surgery(before intervention),7 days after intervention(after intervention),14 days after surgery(after intervention),and 28 days after surgery(after follow-up).The first time of patients getting out of bed was recorded.Statistical analysis was conducted using SPSS.26 to determine the effect of THA early activity program based on social learning theory in the early rehabilitation of hip function in THA patients.The fourth part,research summary and prospect.Results:1.The interaction mode of early activities from environmental support(educator support),individual cognition(self-efficacy,self-regulation,outcome expectation),behavioral response(observational learning,reinforcement learning)and environmental support is constructed and specific contents are formulated.Through expert correspondence consultation,the early THA activity intervention plan based on social learning theory was finally determined,including postoperative health education,psychological assessment and cognitive intervention,model education,wechat resource sharing,activity guidance and supervision,feedback education,selfefficacy intervention and peer support.2.A total of 62 patients with kinetophobia after unilateral THA were included in this study.There were 31 cases in the experimental group,31 cases in the control group,and 1 case in the control group voluntarily asked to quit after discharge.A total of 61 cases were finally included.Before intervention,there was no significant difference in baseline data between the two groups(P> 0.05).After intervention,there were statistical differences in the study indexes between the two groups(P< 0.05),the specific results are as follows:(1)Comparison of rehabilitation self-efficacy scores between the two groups: The scores of rehabilitation self-efficacy of the two groups at different time points showed:time effect(F=65.861,P< 0.001),grouping effect(F=5.845,P< 0.001),interaction effect(F=3.338,P< 0.001),the differences were statistically significant;In addition,the scores of rehabilitation self-efficacy of experimental group were higher than those of control group on day 7(after intervention),day 14(after intervention)and day 28(after follow-up)after surgery(P < 0.05).(2)Comparison of kinetophobia scores between the two groups: The kinetophobia scores of the two groups showed the following effects at different time points(F=143.353,P< 0.001),grouping effect(F=14.671,P < 0.001),interaction effect(F=4.604,P< 0.001),the differences were statistically significant;The kinetophobia scores of the experimental group were lower than those of the control group on the7 th day after surgery(after intervention),14 th day after surgery(after intervention)and 28 th day after surgery(after follow-up)(P < 0.05).(3)Comparison of the first time of getting out of bed between the two groups: the first time of getting out of bed in the experimental group was earlier than that in the control group,and the difference was statistically significant(P< 0.001).(4)Comparison of hip function scores between the two groups: Hip function scores of the two groups at different time points showed: time effect(F=1231.629,P< 0.001),grouping effect(F=38.576,P< 0.001),interaction effect(F=14.870,P< 0.001),the differences were statistically significant;The hip function scores of the experimental group were higher than those of the control group on day 7(after intervention),day 14(after intervention)and day 28(after follow-up)after surgery(P < 0.05).Conclusion:1.Based on the social learning theory,the intervention program of interaction activities at the three levels of individual,environment and behavior can be developed to supplement the current lack of attention to "seeing the relationship and interaction between all parts of the whole from the individual behavior of patients",and provide reference for the formulation of clinical nursing programs.2.The early activity program based on social learning theory can effectively improve the rehabilitation self-efficacy of unilateral THA patients,reduce the level of fear,shorten the first time of getting out of bed activity,and promote the rapid and efficient rehabilitation of patients’ hip function.
