| Objective:The distribution of TCM constitution types of dyslipidemia patients in Kunming area was analyzed,as well as the correlation between blood lipid levels,risk factors(gender,age,drinking,smoking,spicy food,body mass index,etc.)and constitution types.To explore the distribution characteristics of TCM constitutions of patients with dyslipidemia in Kunming area,it will provide a powerful basis for early intervention of this disease from the perspective of constitution and improve the level of clinical diagnosis and treatment of this disease.Methods:From September 2021 to August 2022,A total of 1105 patients who were admitted to Traditional Chinese medicine hospitals in Yunnan province(hospitals in Guanghua Hospital,Dianchi Hospital,and Dianchi Holiday Resort)and received Physical examination and met the inclusion criteria but not exclusion criteria of the urban and rural permanent population of more than 5 years in Kunming,with the age≥18 years and≤80years,and no gender limitation.The TCM constitution classification method of Wang Qi was adopted to identify the constitution,and the general data,body mass index(BMI),blood lipid indexes including triglyceride(TG),total cholesterol(TC),low density lipoprotein(LDL-C),high density lipoprotein(HDL-C),statistical analysis of the above indicators and nine types of constitution(peace quality,qi deficiency quality,yang deficiency quality,yin deficiency quality,blood stasis quality,phlegm-dampness quality,damp-heat quality,qi stagnation quality,characteristic quality),and SPSS26.0 software for data statistics,analysis of the physical distribution characteristics of patients with dyslipidemia in Kunming area and the correlation of lipid indexes.Results:1.Among the 1105 patients with dyslipidemia included,105 cases(10%)were of compound constitution group and 1000 cases(90%)were of simple constitution group.Among the 1000 patients with dyslipidemia with simple constitution in Kunming area,the top three proportions of each constitution type were mild constitution 464 cases(46.4%),yang deficiency constitution 198 cases(19.8%)and qi deficiency constitution95 cases(9.5%).2.Among 1000 patients with dyslipidemia of simple constitution in Kunming area,409cases had simple abnormality of blood lipid index,of which,the abnormalities of TG and TC were the main ones,196 cases(19.6%)had abnormal TG,and 116 cases(11.6%)had abnormal TC;378 cases had two abnormal blood lipid indexes,among which,TC and LDL-C were the most abnormal ones,192 cases(19.2%)in total;There were 200 cases with three abnormal blood lipid indicators,among which,TC,TG and LDL-C were the most abnormal ones,173 cases(17.3%)in total;In contrast,there were 13 patients(1.3%)with abnormal TC,TG,HDL-C and LDL-C.3.The top four types of dyslipidemia were TG↑TC↑LDL-C↑and TC↑TG↑LDL-C↑and TC↑The distribution of these four types of constitution accounted for the top three ones,which were of mild constitution,yang deficiency constitution and qi deficiency constitution.4.Among 1000 patients with simple constitution dyslipidemia in Kunming area,356 cases(35.6%)were male patients,and 644 cases(64.4%)were female patients.There was significant statistical difference between gender and TCM constitution(P<0.05).5.Among 1000 patients with dyslipidemia with simple constitution in Kunming area,217were male without drinking alcohol and 139 were female with drinking history.χ~2test between constitutions and drinking history in male showed statistical significance(P=0.000<0.05);Among the female population,622 cases had no history of drinking alcohol and 22 cases had history of drinking alcohol.χ~2test of the constitutions and drinking history in the female population showed no statistical significance(P=0.169>0.05).6.Among 1000 patients with dyslipidemia with simple constitution in Kunming area,166were male patients without history of smoking and 190 were female patients with history of smoking.X~2test between constitution and smoking history in male population showed no statistical significance(P=0.358>0.05);Among the female population,616 patients had no history of smoking and 28 patients had a history of smoking.χ~2test between the constitutions of the female population and the smoking history showed no statistical significance(P=0.573>0.05).7.Among 1000 patients with simple constitution dyslipidemia in Kunming area,there were significant differences in age,BMI and HDL-C among different constitution types(P<0.05).8.Among 1000 patients with simple constitution dyslipidemia in Kunming area,there was no significant difference in spicy food intake,TC,TG and LDL-C among different constitution types(P>0.05).9.The results of the correlation regression analysis of lipid indicators among the body types were as follows:the differences in cholesterol between qi deficiency,yang deficiency,phlegm-damp,damp-heat,qi deficiency,blood stasis,special endowment and phlegm-dampness were statistically significant(P<0.05),and the likelihood of elevated cholesterol was significantly higher in qi deficiency than in phlegm-dampness(OR=7.293,95%CI:1.207-44.053),phlegm-damp(OR=14.435,95%CI:2.627-79.315),and damp-Heat(OR=57.694,95%CI:10.296-323.305).Cholesterol was lower in yang deficiency(or=0.028,95%CI:0.004-0.222),qi deficiency(OR=0.018,95%CI:0.002-0.194),blood stasis(OR=0.011,95%CI:0.001-0.118),and cholesterol was lower in the extra ordinary quality compared to the peaceful quality(OR=0.003,95%CI:0.000-0.059).The differences of triglyceride increase between qi deficiency,yang deficiency,phlegm-damp,qi deficiency and phlegm-dampness were statistically significant(P<0.05),with yang deficiency significantly more likely to have elevated triglycerides than phlegm-dampness(OR=24.965,95%CI:3.109-200.489)and qi deficiency significantly more likely to have elevated triglycerides than phlegm-dampness(OR=23.001,95%CI:2.063-256.382).Triglycerides were lower in qi deficiency compared to phlegm-dampness(OR=0.176,95%CI:0.033-0.926),phlegm-damp compared to phlegm-dampness(OR=0.022,95%CI:0.004-0.128),and damp-heat compared to phlegm-dampness(OR=0.007,95%CI:0.001-0.039).The difference in high-level and low-level lipoproteins between endowed and calm qualities was statistically significant(P<0.05),and the likelihood of elevated LDL cholesterol was higher in endowed than in calm qualities(OR=5.441,95%CI:1.309-22.605).The difference in HDL between phlegm-damp,there were significant differences between damp-heat syndrome and phlegm-dampness syndrome(P<0.05),with phlegm-damp being less likely to have lower HDL cholesterol than phlegm-dampness(OR=0.272,95%CI:0.085-0.874)and damp-heat being less likely to have lower HDL cholesterol than phlegm-dampness(OR=0.223,95%CI:0.06-0.824).Conclusion:In the study of 1000 patients with dyslipidemia with simple constitution in Kunming area,among the biased constitutions,the constitutions with yang deficiency,qi deficiency and yin deficiency accounted for the most.Among the nine constitution types,there were significant differences in terms of age,BMI,HDL-C,and drinking history of male population,but there were no significant differences in the drinking history of female population,smoking history of male and female population,and spicy food intake,TC,TG,and LDL-C.compared with the mild constitution,the constitutions with higher possibility of cholesterol increase were of qi deficiency,phlegm-dampness and dampness-heat;The constitutions with higher Possibility of triglyceride increase were yang deficiency constitution and qi stagnation constitution.The more likely increase in LDL is intrinsic.In general,the biased constitution situation of patients with dyslipidemia should be adjusted early,and the early management for the population with biased constitution should be continuously strengthened,so as to reduce the prevalence of the disease and the development rate of the disease. |
