Community Home Care Considering Bilateral Satisfaction Research On Medical Resource Scheduling

In recent years,with the aging of China’s population and changes in family structure,the development potential of home care services is gradually emerging.However,the shortage of medical and nursing resources and high labor costs have raised the threshold of senior care services,resulting in some elderly people’s needs not being met;and the scheduling of nursing staff,as an important link in the delivery of community senior care services,directly affects the service efficiency and operating costs of senior care institutions.Therefore,under the current situation of scarce medical and nursing resources,a key issue facing the home care industry is how to optimize the service efficiency of nursing staff to reduce the dispatching cost and improve service satisfaction.At the same time,the planning and scheduling of caregiver resources in a concentrated community can also be applied to the situation when caregivers are needed for home care services in case of major social security problems,so it is of great theoretical and practical significance to study the scheduling of caregiver resources.Considering that the satisfaction of the elderly is the core driving force for the sustainable development of home care in the community elderly service system,and that the conflict between supply and demand is still the main problem in the construction of the elderly service system in China,improving the satisfaction of caregivers is a stable attraction mechanism.Therefore,this paper establishes the bilateral satisfaction of caregivers and the elderly as the main objective function,and introduces the constraints of fuzzy time window,service time and designated appointment personnel,and quantitatively models the satisfaction of the elderly and caregivers respectively through prospect theory;secondly,based on the uncertainty of medical care service time and the variable number of elderly appointments,two kinds of single-assignment determined service time and multi-assignment uncertain service time are established.The second model is based on the deterministic service time and the number of elderly appointments.For the above two models,the multi-objective evolutionary algorithm based on Chebyshev decomposition is used to solve them by designing four operators and variable neighborhood operation.For the single-assignment deterministic service duration model,the pareto frontier distribution is analyzed for different service times,appointment time window intervals and time window types,and the appointment setting form that maximizes bilateral satisfaction is determined in the model.For the multi-assignment uncertain service length model,firstly,we analyzed various parameters in the uncertain service time opportunity constraint model,and secondly,we analyzed the effects of multi-assignment and single-assignment,as well as nurse human resource allocation on bilateral satisfaction from two macroscopic and microscopic perspectives,respectively.The following conclusions were obtained:(1)The time interval of the elderly appointment window is set to 30 min,which can balance the bilateral satisfaction better.(2)Community elderly care institutions should focus on improving the service level of caregivers,and try to avoid the situation that the service level of similar caregivers is too different;(3)the fuzzy time window is more suitable for today’s demand segmentation model;(4)the number of bookable staff is proportional to the uniformity of pareto front distribution,and decision makers should set the number of bookable staff flexibly according to the demand range(seasonal change);(5)under the condition that the demand of the elderly constant,increasing the number of caregivers can significantly improve the quality of the pareto solution set and bilateral satisfaction;(6)decision makers should reasonably set the limit values and confidence levels in the chance constraint model according to different satisfaction preferences.
