| Objective:To verify the clinical efficacy and discuss the mechanism of action by observing the effect of overall adjustment acupuncture(OA)on the iron accumulation status of PMOP patients.Methods:Seventy-six patients with iron accumulation status in PMOP who met the inclusion criteria were randomly divided into 38 cases in the treatment group and 38 cases in the control group.Both groups received oral calcium carbonate D3 tablets(600mg po qd)as the basic treatment,while the control group was treated with alendronate sodium(70mg po qw).The treatment was administered twice a week.The treatment period for both groups was 3 months.Changes in pain scores,quantitative classification of TCM symptoms,PINP,S-CTX,BMD of lumbar spine(L1-L4),SF,serum E2,MDA and SOD were analysed before and after treatment in both groups.Adverse reactions and events that occurred during the treatment period were observed and recorded.The study used SPSS26.0 statistical software to create the data set,collate the data and perform statistical analysis.Results:(1)Baseline analysis:Age(P=0.346),length of menopause(P=0.589),duration of disease(P=0.257),TCM evidence(P=0.640),pain score(P=0.764),TCM evidence quantitative grading score(P=0.326),PINP(P=0.506),S-CTX(P=0.399),lumbar spine(L1 to L4)BMD(P=0.276),SF(P=0.359),E2(P=0.164),MDA(P=0.694)and SOD(P=0.381)before treatment in both groups were statistically analysed and were not significantly different(P>0.05)and were comparable.(2)Intra-group comparison between the treatment group and the control group before and after treatment:(1)Efficacy indexes:there were significant differences between the two groups before and after treatment in terms of pain score,quantitative grading score of Chinese medical evidence,PINP,S-CTX and BMD of the lumbar spine(L1 to L4)(P<0.05).(2)Indicators related to iron accumulation status:there was a significant difference between the two groups before and after treatment in terms of SF in the treatment group(P<0.05)and no significant difference in the control group(P>0.05);there was a significant difference between the two groups before and after treatment in terms of E2,MDA and SOD(P<0.05).(3)Comparison between treatment and control groups after treatment:(1)Efficacy indicators:After treatment,there were significant differences between the two groups in pain score(t=-2.593,P=0.012),quantitative Chinese medicine evidence grading score(Z=-3.914,P<0.001),PINP(t=-2.496,P=0.015),S-CTX(t’=-2.811,P=0.007),and lumbar spine(L1-L4)BMD(t’=2.078,P=0.042)were significantly different(P<0.05).(2)Indicators related to iron accumulation status:there were significant differences(P<0.05)between the two groups in SF(t=-3.165,P=0.002),E2(t=3.727,P<0.001),MDA(Z=-2.381,P=0.017)and SOD(t=5.744,P<0.001)after treatment.(4)Inter-group comparison between the treatment group and the control group in terms of clinical efficacy of TCM symptoms:After treatment,the total effective rate of clinical efficacy of TCM symptoms in the treatment group was 88.57%,while the total effective rate of clinical efficacy of TCM symptoms in the control group was 55.56%;the treatment group was better than the control group in improving the clinical efficacy of TCM symptoms(χ~2=9.560,P=0.002).(5)Safety evaluation:During the treatment process,patients in both groups had stable body temperature,heart rate and other vital signs;no significant abnormalities were found in blood routine,liver and kidney function and other indicators.No serious adverse events occurred in both groups.Conclusion:1.In addition to effectively relieving pain symptoms,improving the quantitative grading score of TCM evidence,reducing P1NP and S-CTX levels to slow down the rate of bone conversion,and improving the efficacy indexes such as BMD of the lumbar spine(L1-L4),OA combined with alendronate treatment can also reduce SF levels to reduce iron accumulation,increase E2 levels in PMOP patients,and reduce It can also lower SF levels to reduce iron accumulation,increase E2 levels and reduce oxidative stress damage in PMOP patients.2.The mechanism for the improvement of iron accumulation in PMOP by OA may be related to the reduction of SF levels and iron accumulation to reduce the high level of oxidative stress induced by it,on the one hand,and the increase of antioxidant E2 levels to reduce oxidative stress damage,on the other. |
