Objective(s):To discuss the function for the expression level of Interferon inducible protein-10 m RNA(IP-10m RNA)in whole blood in pulmonary tuberculosis after mycobacterium tuberculosis antigen stimulationin,and its consistency with Quanti FERON-TB Gold(QFT)detection method.Methods:From March 2022 to February 2023,we collection patients with suspected tuberculosis who were hospitalized in the infectious diseases and hepatology department、respiratory and critical care department of the First People’s Hospital of Yunnan Province,and divided into two groups according to the diagnostic criteria for pulmonary tuberculosis(ws288-2017),and healthy people were recruited as the normal group.All the enrolled people were tested for IP-10.TB.Meanwhile,the results of QFT were collected(excluding the normal group),and the results were compared withtest between the two groups,and the test of Kappa was calculated by the characteristic curve of the subjects.Results:In total,53 patients in the tuberculosis group,23 patients in the non-tuberculosis group,and 8 patients in the normal group were included.Excluding three IP-10.TB uncertainty results in the tuberculosis group,IP-10.TB diagnosis sensitivity was 86%(95%CI:73.8%-93.1%),and specificity in the normal group was87.5%(95%CI:52.9%-99.4%).There was no significant difference between the different diagnosis bases in the pulmonary tuberculosis group of IP-10.TB results(?2=2.134,P=0.362).There was no significant difference of the uncertain results between IP-10.TB and QFT in 53 pulmonary tuberculosis group and 23non-tuberculous group(?2=0.96,P=0.33).Three cases of IP-10.TB test and seven cases of QFT test were excluded,The AUC was 0.74(95%CI:59.8%-88.6%)in IP-10.TB test,The diagnostic sensitivity and specificity were 85.1%(95%CI:72.3%-92.6%)and 63.2%(95%CI:41.1%-80.9%),The AUC detected by QFT was0.87(95%CI:75.9%-97.6%),The diagnostic sensitivity and specificity were89.4%(95%CI:77.4%-95.4%)and 84.2%(95%CI:62.4%-94.5%),There was no significant difference of the diagnostic sensitivity(?2=0.09,P=0.75)and specificity(?2=1.22,P=0.27)of the two test methods.The Kappa value for consistency between the two methods was 0.601(95%CI 30.7%-81.8%,P<0.001).Conclusion(s):IP-10.TB can distinguish between MTB infection,Positive bacteriology,molecular biology,and pathology had no obvious effect on the IP-10.TB test results.There are moderate agreement between the test methods of IP-10.TB and QFT.IP-10.TB is comparable to QFT for PTB diagnostics,faster and simpler operation than QFT,and has good prospect and value for clinical application.
