Objiective:The application of echocardiography to observe changes of right heart function in patients with septic shock and to investigate the prognostic impact of the changes on patients.Methods:Patients with septic shock admitted to the intensive care unit(ICU)from April 2021 to December 2022 were selected.39 patients were finally included according to the definition of sepsis 3.0,and their right heart function-related parameters were recorded by bedside echocardiography on three consecutive days.The general clinical data and the APACHE II score and SOFA score were collected,and the length of stay in the ICU of sepsis patients and their survival at 28 days were followed up and recorded.The impact of changes in right heart function in sepsis on their prognosis was explored by comparing the 28-day mortality rate in the sepsis right heart dysfunction group with that in the normal right heart function group.Results:1.FAC on the first day was significantly different(P<0.05)between the left/right heart dysfunction group and the right heart function normal group,and was significantly lower in the left/right heart dysfunction group than in the right heart function normal group;on the second and third days,it was significantly different(P<0.05)between the independent right heart dysfunction group and the right heart function normal group,and between the left/right heart dysfunction group and the right heart function normal group;2.TAPSE on the first day was significantly different between the independent right heart dysfunction group and the right heart function normal group,and between the left/right heart dysfunction group and the right heart function normal group(P<0.05);on the third day,there was a significant difference between the left/right heart dysfunction group and the right heart function normal group(P<0.05),and the left/right heart dysfunction group was significantly lower than the right heart function normal group;3.The length of stay in the ICU between the right heart dysfunction group and the right heart function normal The length of stay in the ICU was statistically different between the right heart dysfunction group and the normal right heart dysfunction group(P<0.05),and the length of stay was significantly higher in the right heart dysfunction group than in the normal right heart dysfunction group;4.The 28-day mortality rate was higher in the right heart dysfunction group compared with the normal right heart dysfunction patients(P<0.05).Conclusion:Right heart dysfunction is a risk factor for 28-day morbidity and mortality in patients with septic shock,and the length of ICU stay is significantly greater compared to patients with normal right heart function.
