Objective: Vertigo is a common clinical symptom with a high incidence rate.During the attack,patients will have movement hallucinations and balance disorders.At present,with more and more in-depth understanding of vertigo,the number of patients with benign paroxysmal positional vertigo(BPPV)accompanied by severe vertigo symptoms induced by head position changes is gradually expanding.After manual repositioning treatment,this kind of sick people often report dizziness and discomfort,accompanied by balance disorder.Some patients report that such residual symptoms will not be relieved for a long time,or even positional vertigo will recur in a short period of time,affecting the quality of life.The purpose of this study was to observe the clinical efficacy of Fuling Decoction combined with Xiaochaihu Decoction in treating residual dizziness after BPPV reduction,and to provide clinical basis for the treatment of residual dizziness symptoms of BPPV with traditional Chinese medicine.Methods:130 patients with benign paroxysmal positional vertigo who met the enrollment criteria and had residual dizziness after treatment with manual repositioning and were identified by Chinese medicine as phlegm-medium obstruction type were selected.The patients were randomly divided into two groups,65 in each group.Patients in the Chinese medicine group were given Fu Ling Drink combined with Xiao Chai Hu Tang Chinese medicine granules,while patients in the control group were given placebo granules and were given 7 days of medication.The vertigo disorder assessment scale(DHI)was measured after manual reduction and on the 3rd day,the 7th day,the 14 th day and the 28 th day of medication in both groups,and the improvement of clinical signs was compared between the two groups;After repositioning and on the 7th day after taking the medicine,the vertigo clinical syndrome evaluation scale was determined.The data were analyzed by SPSS software.The clinical therapeutic effects of the two groups were objectively evaluated by combining the comparison of experimental results of the two scales.Results:Comparison of DHI scores:(1)Comparison between groups,before treatment,P=0.370>0.05,the difference is not statistically significant;comparison of DHI scores between the two groups of patients in the same treatment cycle,patients in the Chinese medicine group had lower scores(day 3 of treatment,P=0.015<0.05;day 7 of treatment,P=0.012<0.05;day 14 of treatment,P=0.028<0.05;day 28 of treatment(Day 28,P=0.013<0.05),there was a significant difference.(2)In intra-group comparison,the DHI scores of the Chinese medicine group and the control group were compared with the DHI scores before treatment at each follow-up visit(i.e.,day 3,day 7,day 14,and day 28 of treatment),respectively,and the scores were reduced,with significant differences(P<0.001).Comparison of Clinical Evidence Evaluation Scale scores:(1)Comparison between groups,before treatment,P=0.560>0.05,no significant difference;on the 7th day of treatment,the scores of the Chinese medicine group were lower than those of the control group(P=0.006<0.05),a significant difference.(2)Comparing within groups,the scale scores were lower in the Chinese medicine group and the control group after treatment than before treatment(P<0.001),and the difference was statistically significant.The total clinical efficiency was 93.5% in the Chinese medicine group and 76.7% in the control group,and the difference was statistically significant by chi-square test,P<0.05.Conclusion: Compared with the control group,the traditional Chinese medicine group is better than the control group in terms of the improvement of emotion,function,body and other aspects or the improvement of clinical syndromes of traditional Chinese medicine.For patients with benign paroxysmal positional vertigo who have residual dizziness after repositioning and are identified by TCM as phlegm-disturbing type,they can be treated with Fu Ling Drink combined with Xiao Chai Hu Tang with significant effect,further indicating that TCM sutra can effectively reduce the degree of residual dizziness symptoms and confirm the efficacy of TCM.
