| Objective:The paper is designed to evaluate the value of high-frequency ultrasound combined with firefly technique for gout in early diagnosis on the basis of ultrasonic scoring method and compare the differences of seven arthrosis’ s ultrasonography between asymptomatic hyperuricemia and gout patients,and discuss the correlation among gout course,blood uric acid level and ultrasonic scoring.Methods:The cases of 50 patients with gouty arthritis including 100 arthrosis(gout group)and 42 patients with asymptomatic hyperuricemia including 84 arthrosis(hyperuricemia group)were collected in Jilin Province People’s Hospital from December2021 to December 2022,all of the patients had been diagnosed in Rheumatology and Immunology Department and were qualified with the diagnostic,inclusion and exclusion criteria of this study.The general information,relevant clinical data and blood uric acid level of two groups had been recorded,and the double counter sign of the first metatarsophalangeal joint of their feet as well as hyperecho cluster,hyperecho point and bone erosion were be observed by an ultrasound physician under high-frequency ultrasound.The distribution of strong echo points had been observed by high-frequency ultrasound combined with firefly technique.Above symptoms had been scored through ultrasonic semi-quantitative evaluation,and the total score of each arthrosis had been recorded.The difference of ultrasonic score of strong echo points before and after the application of high-frequency ultrasound combined with firefly technique as well as the difference of total score between gout group and hyperuricemia group had been analyzed based on statistics,moreover,the correlation between ultrasonic score and disease course,blood uric acid level in gout group had been further explored.Results:(1)After applying high-frequency ultrasound combined with firefly technique,the score of strong echo point was significantly higher than before(P<0.001),(1.28 ±1.05VS0.65±0.86);(2)Before and after applying high-frequency ultrasound combined with firefly technique,the score of strong echo point in gout group was much higher than hyperuricemia group(P<0.001),(1.10±0.92VS0.12±0.33,1.93±0.86VS0.50±0.69);(3)The detection rate of double counter sign,hyperecho cluster,hyperecho points,synovial hyperplasia,arthroedema,synovial blood flow signals and bone erosion in gout group was obviously higher than hyperuricemia group(p<0.001),(68.0%VS13.1%58.0%VS10.7%,70.0%VS11.9%,58.0%VS33.3%,85.0%VS11.9%,42.0%VS17.9%,42.0%VS10.7%);(4)The total ultrasound score of gouty arthritis was connected with the disease’s duration closely(P<0.001);(5)The total ultrasound score of gout group was significantly higher than hyperuricemia group(p<0.001);(6)There was no obvious correlation between the blood uric acid level of gouty arthritis and the total ultrasound score(P>0.05).Conclusion: 1.The combination of firefly technology improves the detection ability of urate crystals,which has high clinical value for the early diagnosis of gout.2.Asymptomatic Hyperuricemia to gouty arthritis is a continuous pathological process,the new ultrasound scoring method can semi-quantitatively assess the degree of asymptomatic hyperuricemia and gout joint damage. |
