Objective: To compare and observe the clinical efficacy of radiofrequency ablation combined with extracorporeal divergent shock wave in the treatment of discogenic low back pain.Methods: From October 2021 to October 2022,51 patients with discogenic low back pain confirmed by magnetic resonance imaging and discography in the pain department of our hospital were randomly divided into radiofrequency ablation combined with divergent shock wave treatment group(group A)and radiofrequency ablation control group(group B).Among them,25 patients in group A continued to receive extracorporeal divergent shock wave treatment after receiving radiofrequency ablation treatment,once a week,and the treatment period was one month.In group B,26 patients only received radiofrequency ablation.Before each treatment,the visual analog scale(VAS)and the modified ODI score(ODI)were used as pain evaluation indicators.After four weeks of treatment,the modified Mac Nab efficacy evaluation standard was used as efficacy evaluation,and the temperature change of the lesion area was observed with medical infrared thermography as the evaluation index of treatment effect.By comparing the score changes of the two groups before and after treatment,we can judge whether the treatment group has shorter recovery time,more obvious treatment effect,and whether it has the significance of clinical promotion.Results: The scores of patients in Group A and Group B after 1 week of treatment were significantly lower than those before treatment,but after 4 weeks of treatment,the VAS score and the modified ODI score in Group A were significantly lower than those in Group B,the difference was statistically significant(P<0.05);After 4 weeks,the efficacy evaluation results of modified Mac Nab in Group A and Group B were improved.Compared with Group B,the efficacy evaluation standard of modified Mac Nab in Group A was significantly improved in the fourth week(P<0.05).There was no significant difference between Group A and Group B in the temperature zone of infrared thermal imaging before treatment(P>0.05),but after four weeks of treatment,the temperature zone of infrared thermal imaging in Group A was significantly lower than that in Group B(P<0.05).Conclusion: Both groups of patients have obvious effects after treatment.However,from the perspective of short-term efficacy and recovery speed,compared with radiofrequency ablation alone,radiofrequency ablation combined with divergent shock wave therapy for discogenic low back pain is more effective,which can significantly alleviate the symptoms of discogenic low back pain in the short term,and is worthy of further research and promotion.
