Objective: This study was conducted to observe the clinical efficacy of combined Chinese and Western medicine treatment on aquaporin 4 antibody-positive optic neuritis.To explore the positive significance of Chinese medicine in the treatment of such diseases and to provide an objective basis for the clinical treatment of such diseases.Methods: Forty-two patients with a diagnosis of aquaporin 4 antibody-positive optic neuritis,with a total of 43 eyes,who visited the ophthalmology department of Jilin Provincial Hospital of Traditional Chinese Medicine from September 2021 to February 2023,were selected.The patients were randomly assigned to the treatment group and the control group.The Western medicine treatment group used conventional treatment for optic neuritis,while the combined Chinese and Western medicine treatment group added Chinese medicine,acupuncture and moxibustion to the control group,and Chinese medicine treatment was performed according to the syndrome typing of dialectical treatment.The acupuncture points were Zanzhu(double),Sizhukong(double),Sun(double),Yangbai(double),Miming(double),Sibai(double),Chengwe(double),Sanyinjiao(double),Baihui(double),Shenting(double),Hegu(double),Feosanli(double),and Taichong(double).Flat tonification and flat drainage,medium stimulation by manipulation,once a day,with thirty minutes of needle retention.The treatment time was 14 days for both groups.The changes of visual acuity,visual field defect value,visual evoked potential latency and other indicators before and after treatment were used to evaluate the clinical efficacy after treatment,and all data were selected for statistical analysis with the statistical software spss25.0.Results: 1,intra-group comparison between the two groups before and after treatment:visual acuity,P100 wave latency,visual field defect values,and visual function index scores after treatment were statistically significant compared with those before treatment(P <0.05).2,After treatment in both groups,the visual acuity was compared between groups:compared with the Western medical treatment group,the visual acuity in the combined Chinese and Western medical treatment group improved significantly(P < 0.05).3.Inter-group comparison of visual field defect values between the two groups after treatment: compared with the Western medical treatment group,the visual field restored significantly in the combined Western and Chinese medical treatment group(P < 0.05).4.Inter-group comparison of electrophysiological P100 wave latency after treatment between the two groups: compared with the Western medicine treatment group,the combined Western and Chinese medicine treatment group was able to significantly shorten the P100 wave latency(P < 0.05).5.This study found that the causative factors of optic neuritis were complex,including1 case of pneumonia,3 cases of sinusitis,1 case of multiple sclerosis,2 cases of dry syndrome,1 case of upper respiratory tract infection,2 cases of dental pulpitis,and 30 cases without clear causative factors.6 Comparison of clinical efficacy: combined Chinese and Western medicine treatment group: 3 eyes were cured,6 eyes were effective,8 eyes were effective,and 5 eyes were invalid,with a total effective rate of 68.2%;Western medicine treatment group: 4 eyes were cured,8 eyes were effective,7 eyes were effective,and 2 eyes were invalid,with a total effective rate of 76.2%.The efficacy of the combined Chinese and Western medicine treatment group was better than that of the Western medicine treatment group.Conclusion: Both conventional hormone and immunosuppressive western medicine therapy and combined Chinese and Western medicine therapy can effectively treat aquaporin 4 antibody-positive optic neuritis.The clinical efficacy of the combined Western and Chinese medicine therapy was more significant than that of the Western medicine therapy alone.The prevalence rates of the following TCM types in the TCM treatment group were 33.333 for Qi stagnation and blood stasis,14.285 for phlegm and blood stasis,23.810 for Yin deficiency and fire,and 28.571 for liver and kidney deficiency.The advantages of combined Chinese and Western medicine treatment may be achieved by restoring visual acuity,visual field,and shortening the time of visual evoked potentials.
