| Objective: The purpose of this study is to analyze and broaden clinical treatment methods for uric acid nephropathy(syndrome of damp-heat accumulation)by reviewing medical records,and prove the efficacy and safety of Shugan lowering uric acid formula for the treatment of uric acid nephropathy.With the support of data,the new idea of clinical treatment of uric acid nephropathy(syndrome of damp-heat accumulation)is expanded.Methods: Through the retrospective study of 101 patients who were diagnosed as uric acid nephropathy by western medicine and ’ bi disease ’ or ’ lumbago ’ by traditional Chinese medicine in the Affiliated Hospital of Changchun University of Traditional Chinese Medicine from January 2021 to November 2022,and diagnosed as damp-heat accumulation syndrome by traditional Chinese medicine,53 cases were divided into observation group and 48 cases in control group.The observation group was treated with Shugan lowering uric acid formula combined with febuxostat tablets,while the control group was treated with febuxostat tablets.The above-mentioned cases were treated continuously for 4 weeks on the basis of routine treatment.The changes of 24-hour urinary protein quantification(24h-UPQ),serum uric acid(UA),serum creatinine(CR),urea nitrogen(BUN),alanine aminotransferase(ALT),aspartate aminotransferase(AST)and other clinical indicators of western medicine and Traditional Chinese Medicine(TCM)clinical syndrome scores before and after treatment were compared.The efficacy of patients before and after treatment was analyzed by statistical methods,and the conclusions were drawn.Results: The SPSS 26.0 statistical system was used to perform relevant medical statistics on the collected data.The results are as follows : After treatment,there was no significant difference between the two groups in the symptoms of lumbar and knee weakness,fatigue,edema,abdominal distension,and dry stool(P > 0.05).There were significant differences in the symptoms of heavy limbs,bitter mouth,burning urine and diarrhea(P < 0.05).The total score of TCM symptoms was compared within the group and between the groups,P < 0.05,and there were significant differences within the group and between the groups.In terms of TCM symptoms,the effective rate of the control group was87.5 %,and the effective rate of the observation group was 90.57 %.There was a significant difference within the group(P < 0.05).There were significant differences between the two groups before and after treatment in 24 h urine protein quantification,serum uric acid,serum creatinine and urea nitrogen(P < 0.05).There was no significant difference in 24 h urine protein,serum creatinine,alanine aminotransferase and aspartate aminotransferase between the two groups after treatment(P > 0.05).There were significant differences in blood uric acid and urea nitrogen after treatment(P < 0.05).The total effective rates of uric acid nephropathy in the control group and the observation group were 89.58 % and 92.45 %,respectively.There was a significant difference between the two groups(P < 0.05).The observation group has a significant effect on the treatment of uric acid nephropathy(damp-heat accumulation syndrome),and is superior to the control group in improving TCM syndromes such as heavy limbs,bitter mouth,burning urine,loose stools and diarrhea,and reducing blood uric acid and urea nitrogen.Conclusion: The clinical efficacy and TCM syndromes of Shugan lowering uric acid formula combined with febuxostat tablets in the treatment of uric acid nephropathy(damp-heat accumulation syndrome)are more advantageous than the treatment of febuxostat tablets alone.Compared with patients treated with febuxostat tablets alone,Shugan lowering uric acid formula combined with febuxostat tablets can effectively reduce the western medicine indexes of blood uric acid and urea nitrogen,and improve the clinical symptoms of traditional Chinese medicine such as heavy limbs,bitter mouth,burning urine and loose stool. |
