| Objective: To explore and analyze the distribution of pathogenic bacteria and the characteristics of antibiotic sensitivity spectrum in patients with urogenic sepsis caused by upper urinary tract stones,improve the clinical understanding of pathogenic bacteria of urogenic sepsis caused by upper urinary tract stones,and provide a basis for rational selection of antibiotics to a certain extent.Methods: This study retrospectively analyzed the medical records of patients with sepsis caused by upper urinary tract stone obstruction who were treated in the People’s Hospital of Xinjiang Uygur Autonomous Region from January 2004 to September 2021,collected their gender,age,complications,pathogen culture and drug sensitivity test results,and divided the patients into positive group and negative group according to their pathogen culture results,and used t test and chi-square test for statistical analysis to compare the gender Age,complications,distribution of pathogenic bacteria and characteristics of antibiotic sensitivity spectrum.Results: A total of 215 patients were included in this study,including 76 males and 139 females.151(70.2%)were positive for pathogenic culture,with an average age of 50.6 ± 20.2 years,and 64(29.8%)were negative,with an average age of 52.4 ± 18.7 years.Among the strains with positive pathogenic results,154 strains were isolated,of which 125 were gram-negative(81.2%).The most common pathogen was Escherichia coli(98 cases,63.6%).It was also the most common pathogen among gram-negative bacteria,accounting for 78.4% of gram-negative bacteria.There were 25 cases(16.2%)of gram-positive bacteria.The most common strain of gram-positive bacteria was Enterococcus faecium(Group D),6 cases(3.9%),accounting for 24% of gram-positive bacteria;Fungus was found in 4 cases(2.6%).Imipenem(99.2%)has the highest sensitivity to gram-negative bacteria,and vancomycin(100%)has the highest sensitivity to gram-positive bacteria.There was no significant difference in gender,age and complications between the positive group and the negative group(P>0.05).However,in terms of gender,the proportion of female patients suffering from Escherichia coli(74%)was significantly higher than that of male patients(51%),the difference was statistically significant(P<0.05);In the treatment of Escherichia coli infection,the sensitivity of men and women to antibiotics is also different,and the drug sensitivity rate of women to ceftazidime(70.8%)is also significantly higher than that of men(47.8%),the difference is statistically significant(P<0.05).Conclusion:The main pathogenic bacteria in sepsis caused by upper urinary tract stone obstruction are Gram-negative Escherichia coli.It is very important to summarize and analyze the drug sensitivity of pathogenic bacteria regularly for rational and effective drug use. |
