Observation On The Therapeutic Effect Of Lumbar Oblique Manipulation Combined With Comprehensive Rehabilitation On Lumbar Disc Herniation

Objective:To observe the clinical efficacy of lumbar oblique manipulation combined with comprehensive rehabilitation on lumbar disc herniation(LDH),to clarify the effectiveness and safety of bone-setting manipulation combined with rehabilitation in the treatment of LDH,and to improve the clinical recognition and acceptance of lumbar oblique manipulation.Methods:Sixty patients with LDH who were hospitalized in the rehabilitation department of The Second Affiliated Hospital of Gannan Medical University from December2021 to January 2023 were selected as the study subjects in this thesis.Randomly divided into two groups according to their admission order in the hospital,with 30 cases in each group.The control group was given comprehensive rehabilitation therapy(core training,medium-frequency electrical stimulation,traction),and the observation group was treated with lumbar oblique manipulation on the basis of comprehensive rehabilitation.The treatment period for both groups was 28 days.The VAS score,JOA score,modified Schober method forward flexion value,and straight leg elevation angle were recorded and evaluated in both groups during a 14-day and a28-day treatment,in order to clarify the effects of the two treatment regimens on low back pain,lumbar spine function and lumbar spine mobility;While the effectiveness and safety of the treatment between the two groups were also evaluated according to the efficacy evaluation criteria and safety evaluation criteria on the 28 th day of treatment.The data of this study were statistically analyzed by the software SPSS21.0.Results:1.This study guaranteed no statistically significant differences in gender,age,height,weight,BMI,disease duration,prominent segment and VAS score,JOA score,straight leg elevation angle and modified Schober method forward flexion value before treatment between the two groups(P>0.05),so the two groups were comparable.2.VAS scores: intra-group comparison: after 14 and 28 days of treatment,VAS scores were lower in both groups than before treatment(P<0.05).Between-group comparison: after 14 and 28 days of treatment,the VAS scores of the observation group were lower than those of the control group during the same period(P<0.05).3.JOA scores: intra-group comparison: after 14 and 28 days of treatment,JOA scores in both groups were higher than before treatment(P<0.05).Inter-group comparison: after 14 and 28 days of treatment,the JOA score of the observation group were higher than that of the control group during the same period(P<0.05).4.Modified Schober method anterior flexion value: intra-group comparison:after 14 and 28 days of treatment,the anterior flexion value increased in both groups compared with that before treatment(P<0.05).Intergroup comparison: after 14 and28 days of treatment,the anterior flexion value of the observation group were higher than that of the control group in the same period(P<0.05).5.Straight leg elevation angle: intra-group comparison: after 14 and 28 days of treatment,the straight leg elevation angle increased in both groups compared with that before treatment(P<0.05).Between-group comparison: after 14 and 28 days of treatment,the straight leg elevation angle of the observation group were higher than that of the control group during the same period(P<0.05).6.Evaluation of efficacy: 3 patients were cured,23 patients’ treatment got efficacy,3 patients’ treatment were effective and the treatment on 1 patient was invalid in the observation group,with a total effective rate of 96.6%;1 person was cured,17 people showed efficacy,9 people were effective and 3 people were invalid in the control group,with a total effective rate of 90.0%.The efficacy of observation group was superior to control group(P<0.05).7.Safety Evaluation: there were 2 cases of adverse reactions in the observation group and 1 case of adverse reactions in the control group,there was no significant difference between the two groups(P>0.05).Conclusion:Both lumbar oblique manipulation combined with comprehensive rehabilitation therapy and simple comprehensive rehabilitation therapy can effectively relieve pain,improve lumbar spine function and lumbar spine mobility in the treatment of LDH;lumbar oblique manipulation combined with comprehensive rehabilitation therapy is better and more applicable than simple comprehensive rehabilitation therapy in relieving pain,improving lumbar spine function and mobility,which brings more clinical effectiveness;both lumbar oblique manipulation and comprehensive rehabilitation therapy are safe and worth promoting;lumbar oblique manipulation combined with comprehensive rehabilitation therapy will have valuable clinical application and practice in the treatment of LDH.
