Study On The Correlation Between Depression,Anxiety And TCM Syndrome Types Of Rheumatoid Arthritis

Objective: By studying the current situation of anxiety and depression in patients with rheumatoid arthritis(RA)and its correlation with TCM Syndrome Types and disease activity of RA,we can provide reference for screening RA patients with pathological emotions such as anxiety and depression,so as to improve the level of clinical diagnosis and treatment and better prognosis.Methods: A total of 300 patients with RA were included in this study.The general demographic data were counted.The gender,age,course of disease,TCM Syndrome Types and disease activity index of patients with rheumatoid arthritis were observed by using the disease-related information questionnaire.The anxiety and depression were assessed by Zung’s self rating Anxiety Scale(SAS)and Zung’s self rating Depression Scale(SDS).To analyze the correlation between anxiety,depression and 8 TCM syndrome types of RA and the above indexes.Results: 1 、 Among 300 RA patients,84 cases were accompanied by anxiety symptoms(28%)and 108 cases were accompanied by depression symptoms(36%).2、Gender distribution of RA patients: male: female = 1:2.4.The gender distribution of patients with different RA syndrome types was statistically significant(χ2=13.680,P=0.048).3、The age distribution(χ2=14.941,P=0.029)and course of disease(χ2=33.187,P<0.001)of patients with different RA syndrome types are different.4、The distribution and composition ratio of TCM syndrome types of simple anxiety in RA:cold dampness obstruction = rheumatism obstruction > deficiency of liver and kidney >deficiency of Qi and Yin > deficiency of Qi and blood > phlegm and blood stasis obstruction > damp heat obstruction > blood stasis obstruction;The distribution and composition ratio of TCM syndrome types of simple depression in RA patients: cold dampness obstruction > damp heat obstruction = rheumatism obstruction > blood stasis obstruction > deficiency of liver and kidney = deficiency of Qi and yin = deficiency of Qi and blood > phlegm and blood stasis obstruction;The distribution composition ratio of TCM syndrome types combined with anxiety and depression in RA patients: cold dampness obstruction > rheumatism obstruction > deficiency of both qi and blood >blood stasis obstruction > damp heat obstruction > deficiency of liver and kidney =deficiency of both qi and yin = phlegm and blood stasis obstruction.5 、 There are differences in clinical index levels among patients with different RA syndrome types.There are differences in the number of tendered joints,swollen joints and DSA28 disease activity score in patients with RA complicated with anxiety and depression(P<0.05).6、The SAS score of RA patients was negatively correlated with age and course of disease(χ2=-0.587、-0.525,P<0.001).SDS score was positively correlated with gender,age,course of disease,number of tender joints,number of swollen joints and DSA28(χ2=0.231、0.718、0.440、0.374、0.371,P<0.001).Conclusion: The incidence rate of depression and anxiety in RA patients is higher than that in the normal population.RA patients with younger onset age and shorter course of disease are more likely to have anxiety state.Older onset age,longer course of disease,more tendered joints,more swollen joints and higher DSA28 value are more likely to have depression state.The state of depression and anxiety in RA patients is related to the type of TCM syndrome and disease activity.The state of depression and anxiety will affect the prognosis and quality of life of RA patients.
