Objective :(1)Through the research,we can provide the real and reliable scientific basis for the clinical research and application of traditional Mongolian bone-setting manipulation combined with 3d-printing technology in accurate reduction of distal radius fracture.(2)The preliminary exploration and application of Mongolian traditional bone-setting technique combined with 3D printing technology,which provides scientific basis for later researchers.Methods : Sixty cases meeting the diagnostic criteria and inclusion criteria of distal radius extension fracture collected from Hohhot Mongolian and Traditional Chinese Medicine Hospital were randomly divided into two groups,namely the experimental group(reduction by 3D printing technology combined with traditional Mongolian osteopathic manipulation)and the control group(reduction by traditional Mongolian osteopathic manipulation),with 30 cases in each group.Treatment plan: The experimental group used 3D printing technology to print a 1:1 3D model of distal radius fracture,and simulated reduction and fixation on the model.According to the fracture situation shown in the model,traditional osteopathic manipulation of Mongolian medicine was applied for reduction and fixation.After reduction,the 3D model was printed again,and the length,size,thickness and placement of the pressure pad were adjusted in time according to the situation shown in the model after reduction;The control group observed the fracture condition according to the X-ray and CT image data,and used the traditional osteopathic manipulation of Mongolian medicine to reduce the fracture.After reduction,the X-ray and CT images were reviewed again,and the length,size,thickness and placement of the pressure pad were timely adjusted.Both groups were splinted for 21 days and followed up for 8 weeks.Osteopathic reduction was performed by the same chief physician in both groups.Observation indicators:(1)Imaging parameters(palmar Angle,ulnar deviation Angle,distal radius height): 4 weeks before,after and after fracture reduction.(2)Visual analog pain scale(VAS): before and after reduction,3,5,7,11,14 days after reduction.(3)Assessment of Cooney wrist joint Scale: 4 weeks,6 weeks,8 weeks after reduction.(4)Clinical efficacy and the occurrence of adverse reactions were recorded in detail.SPSS25.0statistical software was used to process the observed data.Results : 1.The average age of the two groups was 61.5(49.25,66.0)years old in the experimental group and 55.0(36.75,65.0)years old in the control group,and there was no statistical significance between the two groups(p > 0.05).There was no significant difference in gender between the two groups(p > 0.05).There was no significant difference between the two groups in the affected limb(left \ right)(p > 0.05).2.After the reduction,the effective rate of the experimental group was 96.7% and that of the control group was 93.3%,and there was no statistical significance between the two groups(p > 0.05).3.3.Comparison of VAS index values of pain symptom scores between the two groups showed no statistical significance before reduction between the two groups(p > 0.05),and the two groups could be compared.There was no significant difference between the two groups after reset and 3 days after reset(p > 0.05),but there was significant difference between the two groups at 5,7,11 and 14 days after reset(p < 0.01).4.The comparison of cooney scores of wrist joint function between the two groups showed statistically significant difference 4 weeks after reduction(p< 0.05).6 weeks after reduction,the difference between the groups was statistically significant(p < 0.01).At 8 weeks after reduction,the difference between the groups was statistically significant(p < 0.05).5.Comparison of imaging parameters(palmar inclination Angle,ulnar deviation Angle,radius height)between the two groups after reduction,there was no significant difference between the two groups before reduction(palmar Angle,ulnar deviation Angle,distal radius height)(p > 0.05),and the two groups could be compared.palmar inclination Angle: there were statistically significant differences between the two groups after reduction(p < 0.05);There was statistical significance in 4 weeks after reset between the two groups(p < 0.05).Ulnar deviation: There was statistical significance between the two groups after reduction(p < 0.01).4 weeks after reset,the difference between the two groups was statistically significant(p < 0.01).Distal radius height: There was statistically significant difference between the two groups after reduction(p < 0.05).4 weeks after reset,the difference between the two groups was statistically significant(p < 0.01).7.The rate of adverse reactions after reduction was 0% in both groups.Conclusion: 1.Traditional osteopathic manipulation in Mongolian medicine combined with3 D printing technology can accurately reduce the distal radius fracture,effectively improve the degree of anatomical reduction,and effectively reduce the loss of palmar Angle,ulnar declination Angle and distal radius height in the later period.2.Traditional osteopathic manipulation in Mongolian medicine combined with 3D printing technology can effectively improve the wrist joint function of patients with distal radius fracture in the later period,improve pain symptoms,and improve the clinical cure rate of patients.Therefore,it has good social value and great clinical promotion significance for both patients and medical institutions.
