| Objective: To investigate the current status of psychosocial adaptation of breast cancer patients;to analyze the effects of general information,disease information,self-perceived burden,social support and medical coping style on the psychosocial adaptation status of breast cancer patients.To explore the intervention effect of positive cognitive therapy based on We Chat on the level of psychosocial adaptation and quality of life of breast cancer chemotherapy patients,and to provide an effective method and theoretical basis for clinical improvement of psychosocial adaptation and quality of life of breast cancer chemotherapy patients.METHODS: This study was divided into two parts: a descriptive study and a quasi trial.Descriptive study: A convenience sampling method was used to select 334 breast cancer patients who attended a tertiary hospital in Wuxi from 2021.12 to 2022.8 as the study population.The disease information questionnaire,general information questionnaire,breast cancer psychosocial adjustment questionnaire,self-perceived burden scale(SPBS),medical coping style questionnaire(MCMQ),and social support rating scale(SSRS)were applied to investigate the current psychosocial adjustment of breast cancer patients with breast cancer and its influencing factors.SPSS25.0 statistical software was used to analyze the data.Mean±standard deviation,frequency and percentage were used for statistical description;the effects of general information and disease information on psychosocial adjustment to breast cancer were analyzed by independent samples t-test,one-way ANOVA and non-parametric test;Pearson correlation analysis was explored to the correlation between each variable and the level of psychosocial adjustment.Experimental study: 80 cases of breast cancer chemotherapy patients in a tertiary hospital in Wuxi were randomly selected from 2022.4 to 2022.6,and 40 cases were divided into control group and intervention group.Before and after 2 months of intervention,the patients were evaluated by the FACT-B,MAAS,and the Psychosocial Adjustment Questionnaire for Breast Cancer Patients to analyze the intervention effect of mindfulness-based cognitive therapy.SPSS25.0 statistical software was used to analyze the data.Results:(1)The psychosocial adjustment score of breast cancer patients was 142.63±25.25,which was in the low to medium level overall.(2)The results of univariate analysis showed that the total psychosocial adjustment score was statistically different in general demographic information: age,per capita monthly household income,and medical payment method(P<0.05).There were significant differences in disease-related information: tumor stage,surgical method,treatment plan,number of chemotherapy sessions,and whether metastasis was present(P < 0.05).(3)Pearson analysis showed that: self-perceived burden,coping style,and social support were significantly associated with the total score of psychosocial adjustment of breast cancer patients(P < 0.01).Psychosocial adjustment level should be significantly positively correlated with social support(r=0.542)and confrontation(r=0.689)coping style(P<0.001),and significantly negatively correlated with avoidance coping style(r=-0.502),submission coping style(r=-0.500),and self-perceived burden(r=-0.759)(P<0.001).(4)Multiple linear regression analysis showed that surgical modality,treatment plan,tumor stage,age,monthly per capita household income,self-perceived burden,coping style of confrontation,and social support were predictive factors of psychosocial adjustment,explaining 87.0% of the total variance(F=152.883,P<0.001);social support,use of breast-conserving surgery and breast reconstruction,coping style of confrontation,and household per capita monthly income were protective factors for psychosocial adjustment(P<0.05),while the more treatment options,higher tumor stage,older age,and higher self-perceived burden were associated with poorer levels of psychosocial adjustment(P<0.05).(5)Seventy-seven breast cancer chemotherapy patients completed the interventional study.Before the intervention,patients in the intervention and control groups had mean psychosocial adjustment scores of(144.37±19.26)and(143.69±17.52),mean positive cognitive scores of(49.82±5.29)and(49.56±4.87),and mean quality of life scores of(94.11±10.32)and(93.38±11.21)(6)After the intervention,the psychosocial adjustment scores were(167.74±13.49)and(150.36 ± 15.25)in the intervention group compared to the control group,with the intervention group being higher than the control group(p<0.05).(7)After the intervention,the positive perception scores were(66.29±5.52)and(50.34±3.91)in the intervention group compared to the control group,with the intervention group being higher than the control group(p<0.05).(8)After the intervention,the quality of life scores were(115.82±6.38)and(95.77±9.66)in the intervention and control groups,and the scores were higher in the intervention group than in the control group(P<0.05). |
