Healthy Life Expectancy In The Elderly Based On IADL Criteria

Health is an important basis for the elderly to live on.In previous studies,the health level of the elderly was measured mainly by self-care ability healthy life expectancy.With the improvement of medical technology,the healthy life expectancy of self-care ability has reached a high level in China,but with the continuous improvement of living standards,self-care ability can no longer meet the elderly’s pursuit of a better life.Therefore,this thesis proposes to estimate the healthy life expectancy of the elderly based on IADL health standard,and defines it as the healthy life expectancy of independent living ability of the elderly.The main object of this study is the elderly over 60 years old.Based on the data of China Health and Pension Tracking Survey(CHARLS)in 2013,2015 and 2018,the healthy life expectancy of the elderly in China under ADL and IADL standards was calculated by multi-state life table method.The results showed that there was a significant difference between the healthy life expectancy of self-care ability and independent living ability at the age of 60,in which the healthy life expectancy of selfcare ability was 19.8 years(85.9% of the remaining life expectancy),much higher than the healthy life expectancy of independent living ability(56.9% of the remaining life expectancy).At the same time,the independent living ability healthy life expectancy of male and female,urban and rural elderly groups were measured.The results show that urban and rural and gender differences have a significant impact on the independent living ability and healthy life expectancy.Among them,the average life expectancy of male and female aged 60 is 21.5 years and 25 years respectively,and the healthy life expectancy of independent living ability is 14.4 years and 11.8 years respectively.Although the average life expectancy and healthy life expectancy of male aged people are lower than that of female aged people,However,the independent living ability of elderly men is significantly higher than the healthy life expectancy of women.In addition,the average life expectancy of the 60-year-old urban elderly and the rural elderly is 26.4 years and 22 years respectively,and the healthy life expectancy of the independent living ability is 17.8 years and 11.7 years respectively.The two measurements of the urban elderly are higher than that of the rural elderly,indicating that the situation of the rural elderly is more severe.In order to deeply study the influencing factors of independent living ability and healthy life expectancy of the elderly,16 relevant variables were selected based on the five characteristics of population,social support,health level,economic level and children’s support.logistic regression analysis showed that the health characteristics were the most important factors affecting the healthy life expectancy of the elderly with independent living ability,followed by the demographic characteristics and economic characteristics,and the children’s support characteristics and social support characteristics had the least influence.Among them,the influence coefficient of self-rated health factors was the largest,indicating that the better the results of selfrated health,the longer the healthy life expectancy of the elderly.This thesis proposes for the first time to measure the healthy life expectancy of the elderly by IADL standard,and to measure the healthy life expectancy of independent living ability of the elderly.The results show that there are significant gender differences and urban and rural differences in the healthy life expectancy of the elderly with independent living ability.On this basis,the factors affecting the healthy life expectancy of the elderly with independent living ability are further explored,which provides theoretical support for the formulation of pension policy and allocation of pension resources.
