Objective:To observe the effect of Gua Sha by Hu Fu copper acupuncture on itching in atopic dermatitis with accumulated heat in the heart and spleen in childhood,as well as the improvement of skin lesions and sleep in patients,with a view to improving the quality of life of patients with atopic dermatitis with accumulated heat in the heart and spleen in childhood.Methods:Ninety-two patients with cardiosplenic atopic dermatitis were divided into an experimental group and a control group of 46 patients each according to the random number table method.In the control group,0.03%tacrolimus ointment was applied externally to the lesions(twice daily for 4 weeks)on the basis of conventional care,while in the experimental group,Gua Sha was performed on the basis of Hu Fu copper acupuncture(with the meridians of heart meridian of hand lesser yin、lung meridian of hand greater yin、spleen meridian of foot lesser yin).In the first week,a light scraping method was used,once a day,with a scraping frequency of30 times per minute and a scraping time of 30 minutes.The scraping needs to be scraped through from the second week,twice a week in the second week,once a week in the third week,and once a week in the fourth week,with a scraping frequency of 50 times per minute and a scraping time of40 minutes,for a total of 4 weeks of treatment.Pruritus was measured before and 1,2,3 and 4 weeks after the intervention using the Chinese version of the 12-item Pruritus Scale(12-PSS);lesions,pruritus and sleep were compared using the Score of Atopic Dermatitis Index(SCORAD)score,(SCORAD score includes the area and severity of lesions,VAS pruritus,VAS sleep,)measured before and 2 and 4 weeks after the intervention;quality of life Quality of life index(CDLQI)of childhood dermatology was used and measured before and 4 weeks after the intervention.Results:1.Comparison of general information(1)The differences between the two groups of patients in terms of gender,disease duration and age were found to be statistically insignificant by chi-square test and t-test of two independent samples(P>0.05);(2)The 12-PSS score and SCORAD score before intervention in the two groups of patients showed no statistically significant differences by two independent samples t-test(P>0.05).2.Comparison of pruritus(1)Before the intervention,there was no statistically significant difference between the experimental group and the control group in terms of 12-PSS scores(P>0.05);after 1-4 weeks of intervention in both groups,the 12-PSS scores were lower than before the intervention,and the difference was statistically significant(P<0.05);and after 1,2,3 and4 weeks of intervention,the scores in the experimental group were lower than those in the control group,and the difference was statistically significant(P<0.05);(2)The main effect of time,the interaction effect of group and time,and the main effect of group in the 12-PSS scores of patients in both groups were significant(P<0.05),and the differences were statistically significant.3.Comparison of atopic dermatitis score index(1)There was no statistically significant difference in SCORAD scores between the two groups before intervention(P>0.05),and the SCORAD scores of patients in both groups were significantly lower than those before intervention in 2W and 4W after intervention,and the SCORAD scores in 4W after intervention were significantly lower than those in 2W after intervention,and the differences were statistically significant(P<0.05).The SCORAD scores of 2W and 4W after intervention in the experimental group were significantly lower than those of the control group,and the differences were all statistically significant(P<0.05);(2)The differences in skin lesion area and severity score of 2W before and after intervention were not statistically significant between the two groups(P>0.05),and the skin lesion area and severity score of 2W and4 W after intervention were significantly lower than those before intervention in both groups,and the skin lesion area and severity score of 4W after intervention were significantly lower than those of 2W after intervention,and the differences were statistically significant(P<0.05).The area of skin lesion and severity score of 4W after intervention in the experimental group were significantly lower than those in the control group,and the differences were statistically significant(P<0.05);(3)There was no statistically significant difference in VAS pruritus scores between the two groups before intervention(P>0.05),and the VAS pruritus scores of 2W and 4W after intervention were significantly lower than those before intervention in both groups,and the VAS pruritus scores of 4W after intervention were significantly lower than those of 2W after intervention,and the differences were statistically significant(P<0.05).The VAS pruritus scores of 2W and 4W after intervention were significantly lower than those of the control group in the experimental group,and the differences were all statistically significant(P<0.05);(4)There was no statistically significant difference in VAS sleep scores between the two groups before intervention(P>0.05),and the VAS sleep scores of patients in both groups were significantly lower in 2W and 4W after intervention than before intervention,and the VAS sleep scores of 4W after intervention were significantly lower than those of2 W after intervention,and the differences were all statistically significant(P<0.05).The VAS sleep scores in the experimental group were significantly lower than those in the control group at 2W and 4W after the intervention,and the differences were all statistically significant(P<0.05).4.Comparison of quality of life: before the intervention,there was no statistically significant difference between the CDLQI scores of patients in the experimental group and the control group(P>0.05);after4 weeks of intervention,the CDLQI scores of patients in both groups decreased,and the difference was statistically significant(P<0.05);and the CDLQI scores of patients in the experimental group were lower than those in the control group after 4 weeks of intervention,and the difference was statistically significant(P<0.05).Conclusion:1.Hu Fu copper acupuncture Gua Sha can effectively relieve pruritus in childhood patients with atopic dermatitis with heat accumulation in the heart and spleen.2.Gua sha can significantly improve the area and severity of skin lesions,itching and sleep conditions in children with atopic dermatitis with heat accumulation in the heart and spleen.3.Gua sha can improve the quality of life and satisfaction of children with atopic dermatitis with heat accumulation in the heart and spleen.
