Objective:This paper aims to analyze the distribution of TCM syndromes of maintenance hemodialysis patients in two TCM hospitals in Nanchang area,and analyze the relevant influencing factors of anxiety and depression,based on the data collected from patients with anxiety and depression in maintenance hemodialysis,so as to provide ideas for clinical treatment.Materials and methods: Using the cross-sectional bicenter survey method,a questionnaire survey was conducted in the hemodialysis room of Jiangxi University of Traditional Chinese Medicine and Hongdu Hospital of Traditional Chinese Medicine from December 2021 to December 2022,and the general information(including name,gender and age,etc.)and recent clinical test results(including serum albumin(ALB),were collected.Hemoglobin(HB),Serum creatinine(Scr),blood urea nitrogen(UREA),blood phosphorus(P),parathyroid hormone(PTH),through chronic kidney disease TCM TCM Chronic Kidney Disease TCM Syndrome Differentiation Scale and Hamilton Anxiety and Depression Scale(HAD),TCM symptoms and HAD scores were obtained.SPSS26.0 software was used to analyze the correlation between the patient’s anxiety and depression degree and TCM symptoms,general information and clinical test results.Results:1.The prevalence of anxiety in this survey was 35.0%,the prevalence of depression was 37.2%,and the prevalence of patients with combined anxiety and depression was 27.4%.2.There was a significant correlation(p<0.05)between economic status,child status,and anxiety score,while there was a general correlation(p=0.052)between gender.There was no statistical significance(p>0.05)in the degree of anxiety among individuals,such as work status,age,duration of hemodialysis,education,marriage,financial source,and social status.3.There was a significant correlation(p<0.05)between gender,social status,economic source,and depression score.There was a general significance(p=0.054)between hemodialysis time,children’s condition,and depression score.There was no statistical significance(p>0.05)between age,work status,economic status,education,and marriage in terms of depression level.4.There were statistical differences in anxiety scores,PTH values,and blood P values among MHD patients,with a negative correlation(r=0.54,P<0.01,r=0.692,p<0.01).However,the differences in anxiety scores among HB,ALB,CREA,and UREA were not statistically significant(p>0.05).5.The difference in depression score between MHD patients and PTH value and blood P value was statistically significant and positively correlated(r=0.117,p<0.05),while the difference in depression score between HB,ALB,CREA,and UREA was not statistically significant(p>0.05).6.This deficiency syndrome accounts for 64.6%(51 cases)of the total79 MHD anxiety patients,among which spleen and kidney qi deficiency>liver and kidney yin deficiency>spleen and kidney yang deficiency>qi yin deficiency>both yin and yang deficiency.Standard syndrome accounted for35.4%(28 cases)of the total 79 MHD anxiety patients,among which dampness turbidity syndrome>blood stasis syndrome>dampness heat syndrome>heat toxin syndrome>wind movement syndrome.7.This deficiency syndrome accounts for 65.5%(55 cases)of the total84 MHD depression patients,among which spleen kidney qi deficiency/liver kidney yin deficiency>spleen kidney yang deficiency>qi yin deficiency>both yin and yang deficiency.Standard syndrome accounted for34.5%(29 cases)of the total 84 MHD depression patients,with dampness and turbidity.Conclusion:1.The economic situation of MHD patients is related to the situation of their children and the degree of anxiety.The degree of anxiety of MHD patients with poor economic situation is higher than that of MHD patients with poor economic situation.MHD patients with stable children’s career have lower anxiety than MHD patients with unstable children’s career and no future expectations.There is a certain relationship between the gender of MHD patients and the degree of anxiety.Women have higher anxiety scores than men.MHD patients’ economic sources and social conditions are related to the degree of depression.The degree of depression of MHD patients with independent economic sources is lower than that of MHD patients with independent economic sources.Social frequency was positively correlated with the degree of depression in MHD patients.The condition of children and the duration of hemodialysis have a certain correlation with the degree of depression of MHD patients.The duration of dialysis has a positive correlation with the degree of depression.The degree of depression of MHD patients with stable children’s career is lower than that of MHD patients with unstable children’s career and no future expectations.2.The levels of parathyroid hormone and blood phosphorus in MHD patients were correlated with the degree of anxiety and depression,and the levels of parathyroid hormone and blood phosphorus were positively correlated with the degree of anxiety and depression in MHD patients.3.The degree of depression of MHD patients was more serious in the syndrome of deficiency of liver and kidney and the deficiency of qi and Yin;in the standard demonstration,the syndrome of dampness and turbidity,heat and toxicity and wind movement were more serious;in depression,the deficiency of spleen and kidney Yang and deficiency of both Yin and Yang were the most serious;in the standard demonstration,the depression score was the most serious in the syndrome of congestion and blood. |
