Association Between Chinese Medical Constitution And Related Risk Factors In Atopic Dermatitis In Nanchang Area

Objective: Investigate the bias of TCM constitution in different age stages of patients with atopic dermatitis(AD)in Nanchang,the correlation between TCM constitution and different risk factors for disease,and the correlation between conventional western medicine usage and constitution,to provide guidance and reference for clinical medication,prevention for AD.Methods: The target population is patients who meet the AD diagnosis.A total of 334 cases of AD in the Nanchang region of Jiangxi University of Chinese Medicine Hospital,Jiangxi Provincial Skin Disease Hospital,Jiangxi Provincial Integrated Chinese and Western Medicine Hospital,and the Second Affiliated Hospital of the Jiangxi University of Chinese Medicine were collected.The collected data include general information about patients,related risk factors,regular western medicine glucocorticoid drugs and antihistamine drugs type and duration,syndrome type,and Chinese medicine constitution information.Statistical analysis of the correlation between risk factors and constitution of AD patients in Nanchang region.Results: 1.According to the 334 surveys,the frequencies of Chinese medicine constitution from high to low are: Inherited special Constitution(27.2%)>Yin Deficiency Constitution(21.0%)>Yang Deficiency Constitution(18.0%)>Damp-Heat Constitution(9.3%)> Phlegm-Damp Constitution(7.8%)> Qi Stagnation Constitution(5.4%)> Qi Deficiency Constitution(4.8%)> Balanced Constitution(3.6%)>Blood Stasis Constitution(3.0%).2.The distribution of TCM constitution is different in different age stages,among which patients in the infant are mainly of Inherited special constitution,children are mainly of Inherited special constitution and Yin deficiency constitution,teenagers and adults are mainly of Yang deficiency constitution and Inherited special constitution,and elderly patients are mainly of Yang deficiency constitution.3.Analysis of the correlation between physical condition and various factors:(1)Patients with a lighter weight accounted for 45.4% of the total number of patients,and those with a lighter weight were more often found to Inherited special constitution and Yin deficiency,while patients with BMI more than 23.9were mostly found to be Inherited special constitution,Damp-Heat Constitution and Phlegm-Damp Constitution.(2)Parents with a history of allergic diseases accounted for 65.3% and patients with a history of infantile eczema accounted for 52.6%.Both factors were mostly due to hereditary factors,and the least was the balanced constitution.(3)As for the season of disease aggravation,patients with Inherited special constitution have more disease aggravation in spring,patients with yin deficiency,yang deficiency,and Damp-Heat Constitution have more disease aggravation in summer,patients with yin deficiency constitution have more aggravation in autumn,and patients with yang deficiency and Inherited special constitution have more aggravation in winter.(4)The association between residential proximity to main roads and TCM constitution,29.9% of patients had residential proximity to main roads,of which biased constitution was predominant,and the highest frequency of Inherited special constitution was observed,accounting for 28.3%.(5)40.3% of the patients felt that their quality of life was significantly affected by AD,among which the patients whose quality of life was significantly affected mainly Inherited special constitution,Yin deficiency,and Yang deficiency constitution,while the patients whose quality of life was seriously affected were mainly Inherited special constitution and Qi-stagnation constitution.(6)In terms of sleep,24.5% of patients fell asleep after 11:00 a.m.Among them,patients who fell asleep late were mainly Yang deficiency constitution,Inherited special constitution,and Yin deficiency constitution.(7)56.6% of the patients used glucocorticoid creams for more than one month within six months,among which Inherited special constitution and yin deficiency constitution were more common.(8)The TCM constitution distribution of AD patients did not differ with gender,parental education,infant feeding practices,the floor level of the residence,food allergies,environmental exposure to smoke or pollen,dietary habits,frequency of exercise,or antihistamine medications.4.The distribution of constitution among infants and children was not correlated with gender,parental education,feeding practices,living floors,area of usual residence,environmental exposure to smoke or pollen,dietary habits,sleep status,exercise status,glucocorticoid creams,and antihistamine use.However,there was a correlation between the distribution of constitution and BMI size,parental history of allergic diseases,previous infantile eczema,food allergies,the proximity of usual residence to main roads,daily emotional tendencies,degree of quality of life affected,and season of disease exacerbation.5.The frequency of the TCM types in descending order was: heart fire and spleen deficiency type(28.5%)>spleen deficiency and blood dryness type(21.3%)>wind-damp heat type(21.0%)>spleen deficiency and dampness type(12.1%)>heart and spleen heat accumulation type(11.5%)>spleen and kidney yang deficiency type(5.6%),and there were differences in different constitution.6.Association analysis of common TCM constitution and related factors in AD patients in Nanchang area.Inherited special constitution: The results showed that the season of disease exacerbation and the size of patients’ BMI had statistical significance on the distribution of endowment(P<0.05).Yin deficiency constitution:the results showed that there was a statistically significant effect of a history of eczema in infancy,the season of disease exacerbation,and quality of life affected on the distribution of Yin deficiency constitution(P<0.05).Yang deficiency constitution: The results showed that there was a statistically significant effect of a history of eczema in infancy,the season of disease exacerbation,and daily emotions(P < 0.05).Dampness-heat Constitution:Damp-heat quality: the results showed that there was statistical significance in terms of dietary preferences,the season of onset,and the degree to which quality of life was affected(P <0.05).Conclusion: 1.The top three TCM constitutions in AD patients are Inherited special constitution,yin deficiency,and yang-deficiency.Patients in infancy are predominantly Inherited special constitution,patients in childhood are predominantly Inherited special constitution and yin deficiency constitution,and patients in adolescence/adulthood and old age predominantly yang deficiency constitution.2.In the analysis of the correlation between TCM constitution and various factors,there was a correlation between patient BMI size,parental history of allergic diseases,previous history of eczema in infancy,the season of disease exacerbation,the proximity of the residence to the main road,degree of quality of life affected,sleep status,and topical glucocorticoid medication and TCM constitution,In addition,there were differences in the association between the distribution of constitution and related factors in patients of different ages,and there was a correlation between the distribution of constitution and the characteristics of TCM syndromes.3.Associated between Each constitution and related factors(1)Inherited special constitution: Thin patients with AD in the Nanchang area are more inclined to form Inherited special constitution than normal-weight patients,and the frequency of attack is more pronounced in spring than in summer and winter in patients with Inherited special constitution.(2)Yin deficiency constitution: A history of infantile eczema in AD patients is positively correlated with yin deficiency constitution,and patients with yin deficiency constitution tend to develop the disease more in autumn compared with exacerbation in spring,while patients with low quality of life are less likely to develop yin deficiency constitution compared with patients with less impact on quality of life.(3)Yang deficiency constitution: A history of infantile eczema in AD patients is also positively correlated with the formation of Yang deficiency constitution.Compared with autumn sickness,Yang deficiency patients tend to have winter sickness.In addition,in terms of daily emotions,Yang deficiency patients are more prone to panic than calm patients.(4)Dampness-heat constitution: Compared to patients with no obvious preference for dietary tastes,spicy food is more likely to be dampness-heat constitution,and compared to summer,dampness-heat constitution patients tend to suffer from disease in the spring,in addition,the quality of life is obviously affected by the disease patients are less likely to form dampness-heat constitution.
