The Efficacy Of Percutaneous Patent Foramen Ovale Closure On Migrain Patients Complicated With Paten Foramen Ovale

Objective:Migraine is a primary nerve vascular disease characterized by recurrent episodes of throbbing headache,unilateral or bilateral,which have negative impact on patients’ quality of life.A systematic review demonstrates that compared to the general population,migraine is associated with a higher prevalence of patent foramen ovale.In2004,PFO was found to improve the symptoms of migraine.And research suggests Percutaneous Patent Foramen Ovale Closure can cure or relieve migraine,with success rate as high as 40-80%.However its effectiveness in treatment of migraine remains debatable.This study observed the efficacy of percutaneous PFO closure with esophagus ultrasound-guided in the patent foramen ovale migraine patients and the relationship between the patients with patent foramen form.Method:Patients with migraine complicated with patent foramen ovale(PFO)who were treated in Linyi People’s Hospital from February 1,2020 to January 1,2022,were selected and divided into medical treatment group and interventional closure treatment group,with 60 patients in each group.Medical treatment group receive routine medical treatment.Closure group receive percutaneous PFO closure treatment on the base of medical treatment.We collect and record all of the patients of general information,the related history,imaging information.Headache diary was collected before treatment(T0),1 month after treatment(T1),6 months after treatment(T2),and 12 months after treatment(T3),including headache impact Measurement-6(HIT-6)score,visual analog scale(VAS)score,and attack frequency.According to morphology of foramen ovale,closure group were divided into cystic/tube group and other forms.The relationship between efficacy and foramen morphology were analysed.Results:The HIT-6 score,VAS score,attack frequency were lower than before treatment in two groups.Compared with medical group,the efficacy was more obvious in PFO closure group.In addition,the study also shown that percutaneous plugging treatment in migraine patients with patent foramen ovale,whose patent foramen form was tubular or cystic,is more effective than other patients with patent foramen form.Conclusion:Our preliminary results seen to comfirm that,compared to medical treatment,PFO closure is by far more effective in reducing both attack frequency and degree for migraine patients with foramen ovale.And the efficacy of PFO closure is more obvious for migraine patients whose patent foramen form was tubular or cystic.
