Objective: This study is based on the Herzberg’s two-factor theory to investigate and understand the current situation of physician turnover intention in tertiary public hospitals.It analyzes the effects of health factors,incentive factors,and job satisfaction on turnover intention and their related relationships,identifies the main factors that affect physician turnover intention,and verifies the mediating role of job satisfaction.It proposes effective strategies and suggestions to stimulate physician work enthusiasm,in order to ensure the stability of the physician team in tertiary public hospitals and reduce the turnover rate of physicians.Methods: This study adopts a random sampling method.From August 2021 to September2022,1021 in-service physicians from 6 tertiary public hospitals in Shandong Province were randomly selected for a questionnaire survey.A total of 1042 questionnaires have been collected,and 1021 valid questionnaires were collected,with an effective response rate of 97.98%.The questionnaire mainly consists of four parts,namely the physician basic situation questionnaire,dual factor questionnaire,job satisfaction questionnaire,and turnover intention questionnaire.Statistical analysis methods include descriptive analysis,single factor analysis,multiple linear regression,Spearman correlation analysis,and mesomeric effect analysis.Results: In this study,the turnover intention score of physicians in tertiary public hospitals was lower than the average,with a score of 1.80 ± 0.880.The job satisfaction score was higher than the average,with a score of 4.05 ± 0.871.The descriptive analysis results show that "social recognition","colleague recognition","colleague relationship","hospital level leadership ability",and "social environment" are the five factors with the highest average score.The correlation analysis results show that health factors,motivation factors,and job satisfaction are significantly positively correlated,while turnover intention is significantly negatively correlated;There is a significant negative correlation between job satisfaction and turnover intention.The results of multiple regression analysis showed that physician education,average annual total income,salary situation,work effectiveness,development prospects,job challenges,and hospital level leadership ability had statistical significance(P<0.05)on physician turnover intention.The analysis of mesomeric effect shows that job satisfaction plays a part of mediation role between health care factors,incentive factors and turnover intention.Conclusion: Physicians in tertiary public hospitals have higher job satisfaction and lower levels of turnover intention.The degree of turnover intention varies among physicians with different educational backgrounds,incomes,and salaries.This study established an evaluation index system for physician turnover intention in tertiary public hospitals,and found that work effectiveness,development prospects,job challenges,and hospital level leadership ability are the main influencing factors for physician turnover intention.Moreover,these two factors can affect physician job satisfaction and ultimately affect physician turnover intention.It is recommended that managers of tertiary public hospitals comprehensively evaluate the turnover intention or willingness of physicians through the turnover intention indicator system,analyze the specific reasons for physicians’ turnover intention from different dimensions and perspectives,and take targeted intervention measures. |
