Objective: The risk of stroke in our country ranks first in the world,and triceps spasticity is a common symptom of stroke patients.The occurrence of triceps calf spasticity after stroke can prolong the treatment cycle,increase treatment costs,reduce quality of life,and increase the risk of falls and fractures.This condition will not only have a significant impact on individuals and their family members,but will also impose a significant burden on society as a whole.Now the common methods for treating spasticity include physical therapy,oral drugs,neurolysis technology,surgery,etc.Among them,the effect of physical therapy is short-lived,antispasmodic drugs will cause side effects,neurolysis technology is difficult to operate,and clinical operation is expensive and prone to irreversible nerve damage.For the lack of standard treatment for triceps spasticity after stroke,an effective and non-invasive treatment method is particularly important.Therefore,this study investigated the efficacy of extracorporeal shock wave combined with two different stretching methods on triceps spasm after stroke.Methods: The study recruited 60 patients with triceps spasticity after stroke in the Rehabilitation Department of the Affiliated Hospital of Jining Medical College from March 2020 to August 2021 as the research subjects,and approved by the Ethics Committee of the Affiliated Hospital of Jining Medical College pass.Sixty patients were randomly divided into control group(extracorporeal shock wave group,20 cases),extracorporeal shock wave combined with inclined plate stretching group(combination group A,20 cases)and extracorporeal shock wave combined with static progressive stretching group(combination group B,20 cases),the three groups of patients met the selection criteria,and there was no statistically significant difference in general data.All 3 groups received conventional rehabilitation therapy,among which the control group received shock wave therapy,the combination group A was combined with inclined plank stretching therapy on the basis of the control group,and the combination group B was combined with static progressive stretching therapy on the basis of the control group.The modified Ashworth grading(MAS),modified Tardieu(MTS),simplified Fugl-Meyer lower extremity fraction(FMA-LE)and ankle passive range of motion(PROM)were evaluated before treatment,2 weeks after treatment and 4 weeks after treatment,and the results were statistically analyzed.Results: The functional scores of MAS,MTS,FMA-LE and ankle PROM in the three groups before treatment were statistically analyzed,and the differences were not statistically significant(p > 0.05).After 2 weeks of treatment,there was no significant difference in various assessment indicators of MAS,MTS,FMA-LE,and ankle PROM within the control group compared with those before treatment(p > 0.05),and the assessment indicators of MAS,MTS,FMA-LE and ankle PROM in the combined group A and the combined group B were significantly improved compared with those before treatment,and the difference was statistically significant(p < 0.05);the assessment indicators of MAS,MTS,FMA-LE and ankle PROM in the combined group B were better than those in the control group,and the improvement of ankle PROM in the combined group B was better than that in the combined group A,and the difference was statistically significant(p < 0.05).After 4 weeks of treatment,the three groups of patients were compared within the group,MAS,MTS,FMA-LE,ankle PROM assessment indicators were significantly improved,the difference was statistically significant(p < 0.05);combined group A,combined group B and the control group were compared between the groups,all assessment indicators were better than the control group,the difference was statistically significant(p < 0.05),and combined group B assessment indicators were better than combined group A,the difference was statistically significant(p < 0.05).Conclusion: Extracorporeal shock wave combined with static progressive stretching therapy has a positive therapeutic effect on reducing triceps spasticity and improving lower limb motor function in patients after stroke,and the effect is better than that of combined oblique plate stretching or single shock wave therapy.It can be used as an effective treatment for triceps spasticity after stroke in clinical application.
