| Objective: Fluid attenuation reversal recovery sequence of FLAIR vascular hyperintensity(FVH)is a transient local imaging manifestation with tubular curvature and high signal in subarachnoid space relative to cerebrospinal fluid.However,the influencing factors of FVH are not completely clear.The purpose of this study is to investigate the related factors of FVH formation and grading in patients with cerebral ischemia.Methods: From June 2017 to March 2022,482 patients with cerebral ischemia(277 males and 205 females,with an average age of 62.42±11.08 years)were admitted to our hospital.The baseline clinical data of the patients were collected,and imaging data(including the presence or absence of FVH and FVH score)were analyzed.Then the related factors affecting FVH positive and FVH score were analyzed.Results: 1.449 patients met the inclusion criteria,of whom 73%(328/449)were FVH positive.2.Independent factors influencing the sign of FVH positivity included disease duration,cerebral infarction,degree of internal carotid artery stenosis,and degree of middle cerebral artery stenosis;the longer the disease duration,the lower the rate of FVH positivity,with an OR of 0.613 and 95% CI of 0.445-0.844;patients with cerebral infarction The rate of FVH positivity was higher in patients with cerebral infarction than in those without infarction,with an OR of 5.854 and 95% CI of 3.504-9.778;the more severe the internal carotid artery stenosis,the higher the rate of FVH positivity,with an OR of 1.327 and 95% CI of 1.014-1.737;the more severe the middle cerebral artery stenosis,the higher the rate of FVH positivity,with an OR of 1.523 and 95% CI of1.262-1.737;the more severe the middle cerebral artery stenosis,the higher the rate of FVH positivity.The rate of FVH positivity was 5.606 times higher in patients with severe stenosis-occlusion of arteries(internal carotid and middle cerebral arteries)compared with those with mild stenosis.3.The independent influencing factors of FVH score were the duration of disease,degree of stenosis of internal carotid artery,and degree of stenosis of middle cerebral artery.The longer the duration of disease,the lower the FVH score,with an OR of 0.500 and 95% CI of 0.362-0.691;for the degree of internal carotid artery stenosis,the more severe the stenosis,the higher the FVH score,with an OR of 1.521 and95% CI of 1.238-1.868;for the degree of middle cerebral artery stenosis,the higher the FVH score,with an OR of 1.588 and 95% CI was 1.273~1.980.4.The degree of stenosis was positively correlated with the FVH score,that is,the more severe the stenosis,the higher the FVH score,with a correlation coefficient r of 0.473(P<0.001).Conclusions: 1.The occurrence of FVH is highly correlated with the degree of arterial stenosis in patients with cerebral ischemic stroke.The more severe the degree of vascular stenosis,the higher the incidence of FVH sign.Secondly,the course of disease and whether accompanied by cerebral infarction were also significant influencing factors of FVH sign positive group(P < 0.05).2.FVH score was positively correlated with the degree of vascular stenosis,that is,the more severe the degree of vascular stenosis,the higher the FVH score;In addition,the course of disease also affects the FVH score,which is negatively correlated with the FVH score,that is,the shorter the course of disease,the higher the FVH score. |
