Objective:In addition to participating in hemostasis,fibrinogen(FIB)can also be a key factor in tissue inflammation and repair after intracerebral hemorrhage,which has an important impact on clinical outcome.There have been studies on the occurrence and development of plasma fibrinogen and cerebral hemorrhage.However,the change trend of plasma fibrinogen level at different stages of intracerebral hemorrhage and its relationship with disease progression and prognosis remain unclear.The purpose of this study was to investigate the correlation between the plasma fibrinogen level at different stages and the severity of the disease in patients with cerebral hemorrhage and its predictive value for 30-day prognosis.Methods:Seventy-one patients with spontaneous intracerebral hemorrhage admitted within24 hours of onset from August 2021 to December 2022 were selected as the study object,and physical examination data of 50 healthy individuals in physical examination center of our hospital were randomly selected as the healthy control group.GCS score was performed on admission of all patients,and the patients were divided into mild group(GCS>12 points),moderate group(9-12 points),and severe group(3-8points).At 30 days after onset,the patients were divided into good prognosis group(MRS Score<3 points)and poor prognosis group(MRS Score≥3points)according to the modified Rakin Scale(MRS Score).The plasma FIB level of the control group was compared with that of the groups of the study group at 1,7 and14 days after admission,and the correlation between the plasma FIB level of ICH patients at different time and the severity of the disease and its predictive value for the short-term prognosis were analyzed.Results:The level of plasma FIB in the study group was lower than that in the control group on day 1,and higher than that in the control group on day 7 and day 14(P<0.05).The plasma FIB level in the same group was 7d>14d>1d(P<0.05);At the same time,plasma FIB level in mild group < moderate group < severe group(P<0.05);The plasma FIB level at different times was negatively correlated with GCS score(r=-0.243,-0.706,-0.735,P<0.05),that is,it was positively correlated with the severity of the disease.At the same time,the plasma FIB level of good prognosis group was lower than that of poor prognosis group(P<0.05).The analysis of subjects’ working characteristic curve showed that the plasma FIB level of ICH patients at different time had a certain predictive value for the short-term prognosis,with the highest predictive value on day 7.Conclusion:The level of plasma FIB in patients with cerebral hemorrhage increased with the aggravation of the disease,and gradually decreased after the disease was controlled.The level was positively correlated with the severity and progression of the disease,and had high predictive value for the short-term prognosis of cerebral hemorrhage.
