Objective:1.Based on the theory of lateralization of brain function,this study studied the superior characteristics of lateralization in patients with Schizophrenia(SZ)with AVH using binaural auditory paradigm.Moreover,the difference between the conscious state and the unconscious state is further compared.2.To explore the relationship between brain lateralization advantage characteristics,auditory information processing ability difference between conscious state and unconscious state and AVH symptoms in patients with SZ-AVH.Methods:1.Behavioral study: All subjects were divided into schizophrenia group(SZ-AVH group)and Healthy control group(HC)according to the inclusion criteria.The subjects of the study in the SZ-AVH group were 18 hospitalized patients with SZ-AVH in the Mental Health Department of the First Hospital of Shanxi Medical University,and the HC group were 30 healthy personnel who underwent physical examination.dichotic-listening(DL)paradigm was adopted,2(among subjects: SZ-AVH group,HC group)×3(attention condition: non-forced condition(NF),forced attention to the right ear(forced-right,FR),forced attention to the left ear(FL))×2(threshold value of auditory material: above threshold material,below threshold material)×3(auditory material part of speech: positive word,neutral word,negative word)×2(auricular accuracy: The experimental design of right ear accuracy,left ear accuracy),recorded the responses of subjects to auditory stimuli in the experiment,calculated the accuracy,Lateralization index(LI),attention gain and other indicators.To explore the characteristics of SZ-AVH patients with the advantage of lateralization and the differences between the conscious level of information processing and the unconscious level of information processing for subliminal stimulus materials.2.Correlation analysis: General information such as gender,age,handedness,years of education,etc.of the two groups were recorded respectively,and all scales were evaluated on the day of the behavioral experiment.The demographic difference between the SZ-AVH group and the HC group was compared,and the correlation analysis was conducted for the different indicators,the scores of each scale of the patients with SZ-AVH,and the behavioral experimental indicators of the SZ-AVH group,including accuracy,LI and attention gain Results:1.The correct rate of SZ-AVH group was lower than that of HC group(P=0.024).The material accuracy above threshold was higher than that below threshold(P =0.001).The accuracy rate of positive words was higher than that of neutral and negative words(positive and negative words(P < 0.001),positive and neutral words(P < 0.001),neutral and negative words(P =0.325).2.In NF condition,there was no statistical difference between the right ear accuracy rate and the left ear accuracy rate between the SZ-AVH group and the HC group above and below the threshold(P > 0.05).For the above threshold auditory materials,the right ear accuracy in the SZ-AVH group was significantly lower than that in the HC group(P =0.001)under FR condition,and the left ear accuracy in the SZ-AVH group was significantly lower than that in the HC group(P < 0.001)under FL condition.For subthreshold auditory materials under FR and FL conditions,there was no statistical difference in right ear accuracy and left ear accuracy between the SZ-AVH group and the HC group(P > 0.05).3.The absolute value of LI in the SZ-AVH group was less than that in the HC group(P =0.001)under FR condition,and the absolute value of LI in the SZ-AVH group was less than that in the HC group under FL condition(P < 0.001).4.The gain value of the SZ-AVH group was lower than that of the HC group(P=0.001).There was a statistical difference between FR-gain and FL-gain(P =0.030).5.There was a positive correlation between FR-gain above threshold and positive symptom score(P score),but no significant correlation between FR-gain and the severity of auditory hallucination symptoms(P3 score)and the course of auditory hallucination.Conclusion:1.This experimental study supports the reduction of auditory lateralization advantage in patients with SZ-AVH.The processing of auditory information at the unconscious level is different from that at the conscious level in patients with SZ-AVH.2.Our results suggest that attention gain is associated with overall positive symptoms but not with AVH symptoms.
