Objective:To investigate the expression levels of vascular endothelial growth factor(VEGF)and microvascular density(MVD)in the lamina propria of early gastric cancer specimens with different depths of infiltration, with a view to providing guidance for the diagnosis and treatment selection of early gastric cancer infiltration depth in clinical work. Methods:Patients who met the inclusion criteria(24 cases of early gastric cancer and 23 cases of advanced gastric cancer)attending Fenyang Hospital of Shanxi Medical University from January 2020 to January 2022 were retrospectively collected for study and analysis,as well as normal gastric mucosal tissues from 10 patients with gastritis as controls. They were set up as early gastric cancer group, advanced gastric cancer group and gastritis group respectively, where the early gastric cancer group was divided into intra-mucosal cancer group(T1a)and sub-mucosal cancer group(T1b).Immunohistochemistry was used to detect the expression of VEGF and MVD in each group and to analyse the relationship between the expression of VEGF and MVD in gastric cancer tissues and the clinicopathological parameters of the patients. The value of MVD in predicting the depth of gastric cancer and early gastric cancer infiltration was assessed using the subject operating characteristic curve(ROC).Results:The positive expression of VEGF and MVD gradually increased in the gastritis group, early gastric cancer group and advanced gastric cancer group.the positive expression level of VEGF was not related to gender, age, tumour site, maximum tumour diameter and depth of infiltration in patients with early gastric cancer(P>0.05).However,the level of MVD was related to the maximum diameter and depth of infiltration in patients with early gastric cancer(P<0.05),but not with gender, age, tumour site and tissue differentiation(P>0.05).ROC curve analysis showed that the area of VEGF and MVD under the ROC curve to predict the depth of infiltration of gastric cancer was 0.716(95%CI:0.581~0.828),0.711(95%CI:0.573~0.823);the corresponding sensitivity and specificity were 53.19%and 90.00%,61.70%and 90.00%,respectively.the area under the ROC curve for VEGF,MVD predicting the depth of infiltration of early gastric cancer was 0.596(95%CI:0.414~0.760),0.506(95%CI:0.330~0.681),with a sensitivity and specificity of 29.17%and 90.00%,70.83%and 0.00%,respectively.Conclusion:The positive expression rate of VEGF and MVD level have some predictive value for the depth of gastric cancer infiltration, but the predictive value for the depth of early gastric cancer infiltration is low.
