Objective:To detect serum thyroid hormone levels in patients with acute exacerbation of chronic obstructive pulmonary disease,analyze the prevalence of AECOPD combined with non-thyroid syndrome,and explore the correlation between thyroid hormone and lung function,blood gas analysis,C-reactive protein.Methods:A total of 114 patients with AECOPD admitted to the Department of Respiratory and Critical Care Medicine,Fenyang Hospital,Shanxi Province from October 2020 to October 2022 were selected and divided into 60 patients with NTIS group and 54patients without NTIS group according to whether they had NTIS.The general data between the two groups were compared and the prevalence rate of AECOPD with NTIS group was calculated.Lung function,arterial blood gas and CRP were measured in all subjects.Pearson correlation test was used to analyze the correlation between thyroid hormone and lung function,blood gas analysis and CRP,and Pwas considered statistically significant.Results:1.The prevalence of AECOPD with NTIS was about 53%.There were no significant differences in gender,smoking status,age,BMI,ALB,TG,HDL,PCT,PH,Pa CO2,TSH,TT4,FT4,FEV1,FEV1/FVC and hormone use between NITS group and non-NTIS group(P>0.05).The WBC,NEUT,NEUT%,CRP and length of stay in NITS group were higher than those in non-NTIS group,while the Pa O2,TT3,FT3 and FVC in NTIS group were lower than those in non-NTIS group,and there were statistical differences(P<0.05).2.Pearson correlation test was used to analyze the correlation between thyroid hormone and lung function,blood gas analysis and CRP.TT3 was positively correlated with PH,Pa O2,FEV1,FEV1/FVC(P<0.05).TT3 was negatively correlated with Pa CO2,WBC,NEUT,NEUT%and CRP(P<0.05).FT3 was positively correlated with FVC and FEV1/FVC(P<0.05).FT3 was negatively correlated with NEUT,NEUT%and CRP(P<0.05).Conclusion:(1)NTIS is a common complication during AECOPD.Patients with AECOPD and NTIS had a more pronounced decline in lung function,a stronger inflammatory response and a longer hospital stay.(2)Decreased T3 levels in AECOPD patients were associated with increased CRP,hypercapnia and decreased lung function.(3)TT3 and FT3 measurements may be used as auxiliary indicators to assess the severity of AECOPD. |
