| Objective:Exploring changes in the psychological state of casualties after a disaster and the effectiveness of psychological crisis intervention.Methods:A total of 68 injured persons in the "8.29" collapse accident were collected,and the positive detection rate of Posttraumatic Stress Disorder(PTSD)symptoms and clinical characteristics were analyzed.The patients were assessed with the post-traumatic Stress Disorder Self-rating Scale(PTSD-SS),the simplified Self-rating Anxiety Scale(GAD-7),the Patient Health Questionnaire-9(PHQ-9),the simplified Coping Style Questionnaire(SCSQ),and the Social Support Rating Scale(SSRS).The patients with stress disorder were treated with psychological crisis intervention.The patients were followed up after treatment.The study was followed up and evaluated by a chief physician and an attending physician in the Department of psychiatry.All the subjects were divided into PTSD group and non-PTSD group according to whether PTSD occurred.PTSD was diagnosed according to the criteria of the 5th edition of Diagnostic and Statistical Manual of Mental Disorders(DSM-5)and the duration of symptoms was more than one month.PTSD was screened positive with PTSD-SS score ≥50.Results:1.There were no significant differences in age,gender,marital status,education level,occupation,average personal monthly income,and death of relatives and friends between the PTSD group and the non-PTSD group(P>0.05).2.The decreasing trend of PTSD-SS score in PTSD group was greater than that in non-PTSD group after 1 month,3 months and 6 months of treatment.3.The GAD-7 and PHQ-9 scores of the PTSD group were higher than those of the non-PTSD group at the four time points of before treatment,1 month after treatment,3months after treatment and 6 months after treatment,and the differences were statistically significant(P<0.01).There were significant differences in the group effect and the interaction effect of time effect between GAD-7 and PHQ-9 scores(P<0.01).4.The PTSD-SS score of the PTSD group was negatively correlated with the scores of positive coping,objective support and social support,and positively correlated with the scores of negative coping.The anxiety and depression of the subjects in the non-PTSD group gradually relieved over time.5.After regression analysis of PTSD in all subjects,depression,negative coping and objective support were the influencing factors of PTSD.Conclusion:1.The positive detection rate of PTSD symptoms after this collapse was 39.7%.Although the non-PTSD group did not have PTSD,they had anxiety or depression,and these problems gradually subsided with the passage of time.2.Psychological crisis intervention has an ameliorating effect on PTSD symptoms,anxiety and depression of casualties after encountering a traumatic incident,and can promote the psychological recovery of casualties after a traumatic incident. |
