Objective:To investigate the expression of oncostatin M(OSM)and Apoptosis Inhibitor of Macrophage/CD5L(AOM/CD5L)in patients with sepsis and its role in early recognition of sepsis.Methods:Thirty-four patients(AIM/CD5L:Thirty-five patients)with sepsis admitted in Shanxi Bethune Hospital from June,2021 to January,2022 were selected as a sepsis group,15 patients(AIM/CD5L:18 patients)with community acquired pneumonia(CAP)as a case control group,and 16 adults who underwent physical examination in the same period were selected as a healthy control group.The patients in the sepsis group were followed up for 28 days and divided into a survival group and a death group.SPSS 21.0statistical software was used to analyze the serum OSM and AIM/CD5 L level and their correlation with clinical indexes(leukocyte,neutrophil,lymphocyte count)、sequential organ failure assessment score and acute physiology and chronic health evaluation Ⅱwere analyzed,and the diagnostic value of OSM and AIM/CD5 L expression level in the early identification of sepsis was analyzed.Results:OSM:Compared with the case control group[(368.22±65.95)pg/m L] and the healthy control group[(382.09±73.04)pg/m L],the expression level of OSM[(502.07±209.93)pg/m L] in the sepsis group was significantly higher,with a statistically significant difference(P<0.05).However,the high expression of OSM had no significant correlation with white blood cell,neutrophil,lymphocyte or disease severity score(P>0.05),and there was no significant difference in serum OSM level between the sepsis survival group and the death group.Compared with white blood cell count,the high expression of OSM has certain diagnostic value in the early identification of sepsis.The area under the receiver operator characteristic curve of OSM in predicting sepsis was0.794(95% confidence interval 0.666-0.922,P<0.05),with the sensitivity of 79.4% and the specificity of 73.3%.AIM/CD5L:Compared with the case control group(118.3 ± 12.8)and the health control group(118.7 ± 10.9),the expression level of AIM /CD5 L [127.5(120.2-139.9)]in the sepsis group was significantly higher,with a statistically significant difference(P>0.05).The high expression of AIM /CD5 L was significantly correlated with SOFA score and APACHE Ⅱ score(P>0.05).There was no significant difference in serum AIM /CD5 L levels between the survival group and the death group of patients with sepsis.The area under the receiver operator characteristic curve of AIM /CD5 L for the diagnosis of sepsis is 0.711(95% confidence interval,0.561-0.861,P<0.05),with the sensitivity of 71.4% and the specificity of 66.7%.AIM/CD5 L combined with OSM predicted sepsis with an AUC of 0.895(95%CI,0.761-0.951),a specificity of 100%,but a sensitivity of 60%.Conclusion:The expression of OSM and AIM/CD5 L in patients with sepsis is significantly increased,which may be an effective biomarker for early identification of sepsis and high-risk sepsis patients.
