ObjectiveTo understand the current situation of self management ability of patients with COPD and provide theoretical basis for effective health education;Develop and implement a TCM health education for self management of COPD patients based on the theory of protective motivation,with a view to improving the self management ability and quality of life of COPD patients.Methods1.Cross sectional surveyUsing the convenient sampling method,from January 2022 to June 2022,280 COPD patients were selected from the Department of Pulmonary Diseases of a Class III Hospital in Lanzhou City to conduct a self management ability survey to understand the current situation of self management ability of COPD patients.2.Intervention researchUsing the convenient sampling method,from June 2022 to December 2022,80 patients with COPD in the Department of Pulmonary Diseases of a Class III Hospital in Lanzhou City were selected and divided into a control group and a trial group using a random number table method.The control group and the trial group each had 40 patients.Based on the theory of protective motivation,a Chinese medicine health education for self management of COPD patients was developed and implemented,and the intervention effect was evaluated.Results1.Cross sectional survey results(1)The total score of self management of COPD patients in this study is(154.74 ±23.30),which is at the medium level.After standardizing the scores of each dimension,the highest score for emotional management was(51.44 ± 13.54),while the lowest score for information management was(49.88 ± 15.07).(2)The results of univariate analysis showed that five factors,including the patient’s educational level,occupational type,income level,course of illness,and medical insurance type,affected the self management ability of COPD patients.Multiple linear regression results showed that education level was an influential factor in self management of COPD patients.2.Intervention study results(1)There was no significant difference between the two groups in general information,objective physiological indicators,total self management scores,and COPD evaluation test scores(P>0.05),and the two groups of research subjects were comparable.(2)Repeated measurement analysis of variance results(1)The total score of self management between the two groups of study subjects at different times and in different groups had statistically significant differences(P<0.05)and there was an interaction between time factors and grouping factors(F=89.685,P<0.05).There was no significant difference in the scores between the control group and the experimental group at 3 months(P>0.05).After 6 months of intervention,there was a statistically significant difference in the total scores of self management between the two groups(P<0.05).(2)In the four dimensions of daily life management,information management,emotional management,and self-efficacy,the scores of the two groups of subjects had an interaction between time factors and grouping factors(P<0.05).(3)There was a statistically significant difference in the COPD evaluation test scores between the two groups at different times and between different groups(P<0.05).(4)The respiratory rate,arterial oxygen partial pressure,and arterial carbon dioxide partial pressure of the two groups of subjects had an interactive effect between time factors and grouping factors(P<0.05).ConclusionThe self management ability of COPD patients in this study is at a moderate level.Traditional Chinese medicine health education programs based on the theory of protective motivation can improve the self management ability of COPD patients and improve their quality of life.
