| Objective: By analyzing the cause of death data of Baiyin residents from 2019 to 2021,we can understand the main causes of death and the sequence of the main causes of death affecting Baiyin residents,and the loss of life caused by the cause of death,analyze the main diseases affecting the health of residents,and provide the basis for the formulation of disease prevention and control measures.Through the investigation of death underreporting,the characteristics of death underreporting in Baiyin City are analyzed to provide reference for improving the monitoring of death causes.Methods: The data of death causes in Baiyin City from 2019 to 2021 were collected through the network direct reporting system and public security,civil affairs and medical and health units.The stratified cluster sampling method was used to select samples from townships for underreporting investigation.The death information was obtained by on-site inspection,inquiry and discussion.Compared with the system data,the underreported cases were determined,the death information was sorted out,and the mortality rate,sequence of death causes,life expectancy without death causes,potential years of life lost,potential years of work loss and underreporting rate were calculated.Results:(1)A total of 28,895 deaths were reported in Baiyin City from 2019 to 2021,with a standardized mortality rate of 515.29 / 100,000.The crude mortality rate of males was higher than that of females,and the urban mortality rate was lower than that of rural areas.There are differences in the age of death between different genders.Among the death population,the proportion of death at home,low education,farmers,married and Han nationality was high.(2)The top five causes of death were heart disease,cerebrovascular disease,malignant tumor,respiratory disease and injury,accounting for 84.68 % of all causes of death.The order of urban and rural areas was slightly different.The main causes of death in the 0-year-old group were congenital abnormalities,injuries,and perinatal diseases.The main causes of death in the 5-year-old and 15-year-old groups were injuries.The main causes of death in the 40-year-old group were malignant tumors and heart disease.The main causes of death in people aged 65 and above were cerebrovascular disease and heart disease.(3)The top five death causes of circulatory system diseases such as heart disease and cerebrovascular disease were ischemic heart disease,cerebral infarction,cerebral hemorrhage,other cerebrovascular diseases and other cardiovascular diseases.The mortality rate of male was significantly higher than that of female.The first cause of death in different age groups was ischemic heart disease.Among the top 6 causes of death of malignant tumors(lung cancer,gastric cancer,liver cancer,colorectal cancer,pancreatic cancer,esophageal cancer),5 were digestive tract tumors.The male mortality of breast cancer,cervical cancer,ovarian cancer and uterine body cancer was significantly lower than that of women.The mortality of lung cancer,gastric cancer,liver cancer,esophageal cancer,prostate cancer,bladder cancer and other malignant tumors in males was significantly higher than that in females.The mortality of malignant tumors increased with age.Leukemia is the first cause of death in 0-39 years old,and lung cancer is the first cause of death in people aged 40 and over.The death causes of respiratory diseases were chronic obstructive pulmonary disease,lower respiratory tract infection,asthma and upper respiratory tract infection.The order of death causes of men and women was consistent.Lower respiratory tract infection was the first cause of death in people under 30 years old.The order of people aged 30 and over was consistent.Chronic obstructive pulmonary disease was the first cause of death in people aged 30 and over.The leading causes of injury death were road traffic accidents,accidental falls,other accidental injuries,suicide and sequelae,and drowning.Accidental traffic accidents were the first cause of death for people under 75 years old,and accidental falls were the first cause of death for people aged 75 and over.(4)The average life expectancy in Baiyin City is 79.70 years,and the life expectancy of males(77.96 years)is lower than that of females(81.55 years).Heart disease,cerebrovascular disease and respiratory disease have a great impact on life expectancy.After removing these three diseases,the life expectancy of residents can increase by more than 9years,and the life expectancy of males is higher than that of females.(5)The potential years of life lost(PYLL)of all causes of death in Baiyin City was 128,665 person-years,and the PYLL‰ was 77.40‰.The top three diseases causing the highest loss of life and work were injury,malignant tumor and heart disease.(6)The underreporting rate of the network direct reporting system in Baiyin City from 2019 to 2021 was 2.27%,and the underreporting rate in rural areas was higher than that in towns.The underreporting rate of different age groups was different.The underreporting rate of the 15-year-old group was the highest(20.41%),followed by the 30-year-old group,and the underreporting rate of the 75-year-old group was the lowest(4.48%).Conclusion: The mortality rate of residents in Baiyin City was higher than that of the third national retrospective survey of death causes,and the life expectancy was higher than the national average.Heart disease and cerebrovascular disease were the main causes of death,followed by malignant tumors,respiratory diseases and injuries.Injury is the leading cause of life loss and job loss.The underreporting rate in Baiyin City is lower than that in some parts of the country.Although the underreporting rate is not high,the underreporting still exists.Therefore,it is still necessary to strengthen multi-department communication and cooperation,establish an inter-departmental sharing platform,strengthen the attention of rural and young people,improve the level of data management,the quality and integrity of death cause reports,and improve the death cause monitoring data. |
