Objective:Based on the"Holographic Theory"and"Meridian Theory",this research selected holographic meridian copper needle scraping as an intervention method.By observing the changes in sputum, blood gas, inflammatory factors, and lung function in AECOPD patients with phlegm-heat stagnation syndrome before and after the intervention,it evaluated the expectorant effect, and provided new ideas and methods for efficient expectorant excretion with AECOPD of phlegm-heat stagnation syndrome. Methods:From January 2022 to October 2022,80 patients with AECOPD of phlegm-heat stagnation syndrome who were hospitalized in the Department of Pulmonary Disease of Gansu Provincial Hospital of Traditional Chinese Medicine were randomly divided into a trial group and a control group, with 40 patients in each group, based on the inclusion and exclusion criteria. The control group was given routine treatment and expectorant care, while the experimental group was given holographic meridian copper needle scraping therapy on the basis of the control group.Intervention was conducted every 5 days, the course of treatment is 2 weeks, with a total of 3interventions.Observe the changes of sputum before,1 week after, and after the intervention(including the improvement of sputum output, sputum viscosity)and blood gas analysis indicators before and after the intervention:arterial partial pressure of oxygen(Pa O2),partial pressure of carbon dioxide(Pa CO2);C-reactive protein(CRP);Pulmonary function:forced expiratory volume in one second(FEV1),forced expiratory volume in one second/forced vital capacity(FEV1/FVC%);Changes in COPD evaluation test(CAT)scores and statistical analysis of relevant data.Results:1.General information:There was no significant difference in general demographic data such as gender, age, nationality, education level, occupation, marital status and medical insurance type between the two groups before treatment(P>0.05).2.Clinical data and medication:There was no statistically significant difference between the two groups in terms of clinical characteristics such as course of disease, disease classification, smoking history, types of comorbid diseases, and the use of antibiotics and aerosol drugs before intervention(P>0.05),which was comparable.3.Changes in sputum:(1)Analysis of variance using repeated measurements was used to compare the changes in sputum output between the two groups of patients before,1 week after, and after the intervention. The results showed that there were significant differences in the time effect, grouping effect, and interaction effect of sputum output between the two groups(P<0.05).(2)Sputum viscosity:Compared with before the intervention, the proportion of sputum viscosity I and II in the first week of treatment and after the intervention in both groups increased, and the test group was higher than the control group, with a statistically significant difference(P<0.05).4.Blood gas analysis index:Compared with before intervention,Pa O2and Pa CO2of the two groups were improved after intervention, and the improvement of Pa O2and Pa CO2in the experimental group was significantly higher than that in the control group(P<0.05).5.C-reactive protein(CRP):Compared with before intervention, the CRP of the two groups decreased after intervention, and the decrease of the experimental group was more significant than that of the control group, the difference was statistically significant(P<0.05).6.Pulmonary function:Compared with before intervention, there were significant differences in FVC,FEV1 and FEV1/FVC between the two groups after intervention(P<0.05);There was no significant difference in FVC,FEV1and FEV1/FVC between the two groups after intervention(P>0.05).7.CAT Score:Compared with before intervention, the CAT scores of the two groups decreased after intervention, and the difference was statistically significant(P<0.05).And t he experimental group decreased more significantly than the control group, the difference was statistically significant(P<0.05).Conclusion:Compared with conventional expectoration nursing, holographic meridian copper needle scraping can reduce the sputum viscosity of AECOPD patients with phlegm-heat stagnation syndrome, increase the amount of expectoration, and promote effective expectoration; It can improve the blood gas and inflammatory indicators of patients to a certain extent, improve the quality of life, and is simple and convenient to operate. It has a certain promotion significance, but there is no obvious advantage in improving lung function indicators.
